84例老年类风湿关节炎临床和血清学特征分析

摘　　要：目的探讨老年类风湿关节炎(elderly onset rheumatoid arthritis,EORA)患者的临床特征和实验室检查结果特点。方法回顾性分析84例EORA患者和66例非EORA ((Non-Elderly onset rheumatoid arthritis,NEORA)患者的临床和血清学资料,总结临床特征和实验室检查特点。结果 EORA组与NEORA组比较:EORA组男性患者25例,占比29.8%;髋关节受累患者22例; CRP中位水平为37.5(0.1～393.5) mg/L。EORA组男性构成比、髋关节受累数、CRP中位水平均高于NEORA组,差异有统计学意义(P<0.05)。两组患者在病程、关节肿胀数、关节疼痛数、膝关节受累数、并发间质性肺病数、高滴度类风湿因子、高滴度抗环瓜氨酸肽抗体及疾病活动度DAS28评分方面无统计学差异。结论 EORA患者男性构成比大,髋关节易受累,C反应蛋白水平高。Objective To investigate the clinical features and characteristics of laboratory findings in elderly onset rheumatoid arthritis patients（ EORA）. Methods The clinical and serological data of 84 cases of EORA and 66 cases of non-EORA（ NEORA） were retrospectively analyzed. The clinical and laboratory features were summarized.Results There were 25 cases of male patients in EORA group and account for 29.8% of the patients,22 cases of hip joint involvement,and the median level of C reactive protein was 37.5（ 0.13 ～ 93.5）mg/L. Compared with the NEORA group,the constituent ratio of male patients,implicated number of hip joint and the median level of CRP in EORA group was statistically higher（ P〈0. 05）. There were no significant difference on disease duration,number of joint swelling,number of joint pain,knee involvement,interstitial lung disease,high titer rheumatoid factor,high titer anti-cyclic citrullinated peptide antibody,and disease activity DAS28 score between the two groups. Conclusions EORA group has more male patients than NEORA group,and the hip joints are easy to be affected,the level of C-reactive protein is higher than that in NEORA group.
