摘 要:目的回顾分析普通外科接受胰腺手术治疗合并感染的患者的病原菌构成以及药敏分析报告,以期为胰腺疾病预防性以及治疗性使用抗生素提供临床参考。方法收集136例因胰腺疾病并接受手术治疗的患者体液标本,对标本进行细菌培养,检测菌株对抗生素的耐药情况。结果分离出570株细菌,分离株数前5位分别是大肠杆菌(12.8%)、鲍曼不动杆菌(10.7%),铜绿假单胞菌(9.8%),D群链球菌(8.9%),金黄色葡萄球菌(8.8%)。耐药试验表明,常用抗生素如β-内酰胺类、氨基糖苷类、四环素类和红霉素已出现普遍的耐药情况,本研究的分离菌普遍敏感的抗生素依次为替考拉宁(100%)、万古霉素(99.6%)、呋喃西林(82.1%)、亚胺培南(77.3%)、美洛培南(75.7%)。结论胰腺手术后感染的细菌耐药菌株数在增加,耐药性在变强。开展细菌耐药性的监测,对指导临床进行预防性和治疗性抗生素的应用具有重要意义。Objective Retrospectively to analyze the reports on bacterial spectrum and resistant strains in the cases of post operation of pancreases. This provides a clinical reference toprophylactic and therapeutic antibiotics for pancreatic disease. Methods Different samples of 136 patients were collected and cultured for bacteria. Then the patterns of sensitivity of the infecting organisms were tested. Results 570 bacterial strains were found, and the positive detection ratein the top five are escherichia coli (12.8%), acinetobacter baumannii (10.7%), Pseudomonas aeruginosa (9.8%), streptococcus group D (8.9%) and staphylococcus aureus (8.8%). Our data demonstrated that most bacterias had been resistant to several sorts of antibiotics, such as β-lactams, aminoglycosides, tetracycline and erythromycin. Several antibiotics including teicoplanin (100%), voncomycin (99.6%), nitrofurazone (82.1%), imepenem (77.3%), meropenem (75.7%) are sensitive to all of the bacterias. Conclusion The bacterial spectrum and resistant strains in the cases of post operation of pancreases increase constantly. Therefore, it is critical to routinely perform the sensitivity test of bacterias.
