摘  要:目的探讨枸橼酸咖啡因治疗极低出生体重儿呼吸暂停的安全性及有效性。方法 2015年1月至2016年12月在我院新生儿科NICU收治的76例出生体重<1500 g,胎龄≤32周的早产儿,分成实验组(n=40)和对照组(n=36),实验组用枸橼酸咖啡因治疗,对照组用氨茶碱治疗,比较两组对呼吸暂停治疗的有效率、副作用及并发症等。结果两组患儿(实验组vs对照组)性别、胎龄、出生体重、1 min及5 min Apgar评分、住院天数差异无统计学意义(P> 0. 05)。两组患儿呼吸暂停发生率[45%(18/40) vs 52. 8%(19/36)]、机械通气时间[(3. 25±3. 28) vs(3. 36±2. 80)]差异无统计学意义(P> 0. 05)。结论枸橼酸咖啡因治疗极低出生体重儿呼吸暂停,安全有效,可有效降低呼吸暂停、支气管肺发育不良、动脉导管未闭及喂养不耐受等合并症的发生。Objective:To compare the efficacy and safety of caffeine citrate on the treatment of apnea in very low birth weight (VLBW) preterm infants. Methods:Totally 76 VLBW preterm infants ,who were enrolled in the NICU of The first people' s Hospital of Foshan from Janurary 2015 to December 2016,were assigned to either experimental group (n = 40) who were treated with caffeine citrate or control group ( n = 36) who were treated with aminophylline. The overall response rates and the complication rates in the two groups were compared. Results:There were no significant difference in male/female ratio, gestational age,birth weight, lminute and 5minute Apgar' s score,hospital day between the two groups (P 〉 0. 05). There were no significant differences in the incidence of apnea145% (18/40) vs 52. 8% (19/36)1, mechanical ventilation timel (3.25± 3.28) vs(3.36 ±2. 80) ](P 〉0. 05). Conclusion: Caffeine citrate is effective on the treatment of apnea and reduces the rate of bronchopulmonary dysplasia, patent ductus arteriosus, feeding intolerance in VLBW preterm infants.
