摘 要:目的:研究肥胖2型糖尿病患者糖化血红蛋白(Hb Alc)、纤维蛋白原(FIB)和血脂指标评估颈动脉狭窄程度的可行性。方法:选取2014年1月至2017年6月145例肥胖2型糖尿病患者,并行颈动脉超声检测,按照超声诊断结果分为正常对照组(A组,49例)及狭窄组(B组,96例)。B组按照狭窄程度分为轻、中、重组,所有患者检测Hb Alc、血脂及FIB。结果:(1) B组患者体质指数(BMI)、病程、血清总胆固醇(TC)、Hb Alc、低密度脂蛋白胆固醇(LDL-C)及FIB高于A组(P <0. 05)。(2)颈动脉狭窄轻、中、重三组中,各组TC、LDL-C以及FIB的差异均有统计学意义(P <0. 05),BMI、病程、Hb Alc及高密度脂蛋白胆固醇(HDL-C)等指标比较差异无统计学意义(P> 0. 05)。(3) TC、LDL-C、FIB与颈动脉狭窄程度均呈正相关(P <0. 05)。结论:颈动脉狭窄的肥胖2型糖尿病患者Hb Alc、FIB和血脂均增高,而FIB和血脂可以间接评估其狭窄程度。Objective: To study the feasibility of assessing carotid stenosis with hemoglobin (HbAlc), fibrinogen (FIB) and blood lipid in obese diabetic patients. Methods: The carotid artery was examined by ultrasound in 145 patients with obese diabetes. They were divided into normal group (group A, 49 cases) and narrow group(group B, 96 cases). The degree of carotid artery stenosis was divided into light, middle, and recombinant. Glycosylated Hemoglobin (HbAlc), fibrinogen (FIB) and blood lipid concentration were detected separately. Results: (1) The Body Mass Index (BMI), the course of disease, serum total cholesterol (TC), HbAlc, low density lipoprotein cholesterin (LDL-C) and FIB in group B were higher than those of group A ( P 〈0.05). (2) In light, middle, and recombinant groups, the differences of TC, LDL-C and FIB in each group were significant ( P 〈0.05). There were no significant differences between the BMI, the course of disease, HbAlc and high-density lipoproteincholesterol (HDL-C) ( P 〉0.05). (3) Correlation analysis showed that TC, LDL-C and FIB were positively correlated with carotid stenosis ( P 〈0.05). Conclusion: The HbAlc, blood lipid and FIB levels are high in carotid stenosis in patients with obese type 2 diabetes, and blood lipid and FIB can be used indirectly to evaluate carotid artery stenosis.
