摘 要:目的:探讨1. 5 T的磁共振成像在胆总管结石诊断价值。方法:选取胆总管结石患者126例,患者均进行双源螺旋CT扫描、1. 5 T磁共振成像检查,比较两种方法的诊断准确性。结果:胆总管结石的CT表现:胆总管内圆形或高密度影,混杂密度影或软组织密度影充满整个管腔,周围无低密度影环绕;结石位于扩张的胆总管腔内,中心呈高或软组织密度影;高密度影结石或软组织密度结石周围被新月形液性密度胆汁围绕。胆总管结石的核磁共振表现:能清晰显示肝内、外胆管及扩张胰管的形态结构;能显示结石所在的部位、数量、形态,表现为腔内卵圆形或圆形充盈缺损;胆总管结石均伴有不同程度的胆总管扩张;胆管梗阻端形态表现为杯口征、充盈缺损或抱球征等。1. 5T磁共振成像组诊断准确率92. 1%,高于双源螺旋CT组的83. 3%(P <0. 05)。结论:1. 5T的磁共振成像可以提高胆总管结石的诊断准确性。Objective: To investigate the diagnostic value of 1.5T magnetic resonance imaging in choledocholithiasis. Methods: 126 cases of choledocholithiasis were chosen and examined with dual source spiral CT scanning and 1.5T magnetic resonance imaging, with the diagnostic accuracy of the two methods compared. Results: CT of choledocholithiasis showed that there were circular or high-density shadows in choledocholithiasis, that the entire lumens were filled with mixed or soft tissue density shadows , surrounded by no low-density shadows , that the stones were located in the dilated common bile ducts, with high or soft tissue density in the center and that high-density shadow stones or soft-tissue-density stones were surrounded by crescent-shaped density bile. Magnetic resonance imaging of choledocholithiasis showed clear morphological structures of the livers, the outer bile ducts and the dilated pancreatic ducts, and the location, the number and the shape of the stones, expressed as oval or round filling defects in the cavity. It also showed choledocholithiasis was associated with varying degrees of common bile duct dilatation and that the morphology of the bile duct obstruction was characterized by cup mouth syndromes, filling defects or holding ball syndromes. The diagnostic accuracy of 1.5T magnetic resonance imaging group was 92.1 %, higher than that of dual source spiral CT scanning group (83.3 %)( P 〈0.05). Conclusion: 1.5T magnetic resonance imaging can significantly improve the diagnostic accuracy of Choledocholithiasis.
