关节镜手术与非手术治疗对踝关节退行性骨关节病的临床疗效比较

摘　　要：目的分析关节镜手术治疗踝关节退行性骨关节病的临床疗效及安全性。方法将90例踝关节退行性骨关节病患者随机分为试验组和对照组,各45例。试验组给予关节镜手术治疗,对照组给予非手术治疗。比较两组的治疗效果。结果治疗后及随访6个月,两组患者的Mazur、NRS、AOFAS和Tegner评分均改善,且试验组均优于对照组(P<0.01)。与治疗后比较,试验组随访6个月各评分均改善(P<0.01)。治疗后,两组贫血、周围神经病变、恶心、腹泻、腹胀、便秘等不良反应发生率无显著差异(P>0.05)。结论关节镜手术治疗踝关节退行性骨关节病的效果优于非手术治疗,可明显缓解患者踝关节疼痛,改善踝关节功能及运动障碍,安全性高。Objective To analyze the clinical efficacy and safety of arthroscopic surgery in the treatment of degenerative osteoarthritis of ankle joint. Methods Ninety patients with degenerative osteoarthritis of ankle joint were randomly divided into experimental group and control group, with 45 cases in each group. The experimental group was given arthroscopic surgery, while the control group was given non-surgical treatment. The therapeutic effects of the two groups were compared. Results After treatment and 6 months follow-up, the scores of Mazur, NRS, AOFAS and Tegner in the two groups improved, and those in the experimental group were better than the control group （P〈0.01）. Compared with after treatment, all the scores in the experimental group improved during the 6 months follow-up （P〈0.01）. After treatment, there were no significant differences in the incidences of anemia, peripheral neuropathy, nausea, diarrhea, abdominal distention and constipations between the two groups （P〉0.05）. Conclusion Arthroscopic surgery for degenerative osteoarthritis of ankle,joint is superior to non-surgical treatment, which can significantly relieve ankle joint pain, improve ankle joint function and dyskinesia, with high safety.
