摘 要:目的:探讨IgM、IgG含量检测对小儿支原体肺炎诊断及病情判断的价值。方法:选择在本院接受住院治疗的支原体肺炎小儿560例作为支原体肺炎组,同期在本院进行疫苗接种的健康儿童200例作为正常对照组。对比两组外周血中IgM、IgG含量及血清中炎症因子、肺表面活性蛋白含量的差异,采用Pearson检验评估支原体肺炎患儿外周血中IgM、IgG含量与炎症因子、肺表面活性蛋白的内在联系。结果:支原体肺炎组患儿外周血中IgM、IgG的含量高于正常对照组;血清中炎症因子MCP-4、IL-5、IL-13、IL-17、IL-18、IL-23的含量高于正常对照组;血清中肺表面活性蛋白SP-A、SP-B、SP-C、SP-D的含量高于正常对照组小儿。相关性分析发现,支原体肺炎患儿外周血IgM、IgG含量与炎症因子、肺表面活性蛋白含量均呈正相关。结论:支原体肺炎患儿外周血中IgM、IgG含量异常增高,与机体炎症反应及肺损伤程度密切相关。Objective: To explore the value of lgM and lgG contents for the diagnosis and judgment of mycoplasma pneumonia in children. Methods: A total of 560 children with mycoplasma pneumonia who received inpatient treatment in our hospital between October 2016 and October 2017 were selected as mycoplasma pneumonia group, and 200 healthy children who received vaccination in our hospital during the same period were selected as normal control group. The differences in peripheral blood lgM and lgG contents as well as serum inflammatory factor and pulmonary surfactant protein contents were compared be tween the two groups, and Pearson test was used to cvaluate the inner link of peripheral blood IgM and IgG contents with inflammatory factor and pulmonary surfactant proteins in children with mycoplasma pneumonia. Results: Peripheral blood lgM and IgG contents of mycoplasma pneumonia group were higher than those of normal control group; serum inflammatory factors MCP-4, IL-5, IL-13, IL-17, IL-18 and IL- 23 contents were higher than those of normal control group; serum pulmonarysur- factant proteins SP-A, SP-B, SP-C and SP-D contents were higher than those of normal control group. Correlation analysis showed that peripheral blood lgM and IgG contents in children with mycoplasma pneumonia were positively correlated with the contents of inflammatory factors and pulmonary surfactant proteins. Conclusion: Peripheral blood IgM and IgG contents are abnormally high in children with mycoplasma pneumonia,and they are closely related to the inflammatory response and lung injury degree.
