摘 要:目的探讨腹膜透析与血液透析对慢性肾脏病5期患者钙磷代谢、生存质量、并发症的影响。方法选取我院收治的慢性肾脏病5期患者100例,随机分为两组各50例。对照组采取血液透析疗法,观察组采取腹膜透析疗法。透析6个月后,比较两组的钙磷代谢水平、各项生化指标、生存质量评分以及透析期间并发症发生率。结果透析后,观察组的血清磷和总胆固醇水平均显著低于对照组(P <0.01),血肌酐、尿素氮水平均显著高于对照组(P <0.01);观察组的日常生活影响(EKD)、睡眠(SL)评分显著高于对照组(P <0.01);观察组的心力衰竭、肌痉挛、消化道出血发生率均显著低于对照组(P <0.05)。结论与血液透析相比,腹膜透析治疗慢性肾脏病5期患者可进一步改善钙磷代谢紊乱、生化指标和生存质量,且部分并发症的发生率更低。Objective To investigate the effects of hemodialysis and peritoneal dialysis on the calcium and phosphorus metabolism and life quality of patients with stage 5 chronic kidney disease (CKD). Methods 100 cases of patients with stage 5 CKD admitted to our hospital were randomly divided into two groups equally. The control group was treated with hemodialysis, while the observation group was treated with peritoneal dialysis. After 6 months of dialysis, the levels of calcium and phosphorus metabolism, biochemical markers, scores of life quality, and incidence of complications during dialysis were compared between the two groups. Results After dialysis, the levels of serum phosphorus and total cholesterol of the observation group were significantly lower than those of the control group (P 〈0.01), and the levels of serum creatinine and urea nitrogen were significantly higher than those of the control group (P〈0.01). The scores of effects of kidney disease on daily life (EKD) and sleep (SL) of the observation group were significantly higher than those of the control group (P〈0.01). The incidences of heart failure, tendon and gastrointestinal bleeding of the observation group were significantly lower than those of the control group (P 〈0.05). Conclusions Compared with hemodialysis, peritoneal dialysis for the treatment of patients with stage 5 CKD can better improve the disorder of calcium and phosphorus metabolism, as well as the biochemical indicators and life quality, and has lower incidence of some complications.
