摘  要:目的探讨血清(1, 3)-β-D-葡聚糖检测在深部真菌感染诊断中的应用价值。方法选取2014年9月至2018年3月在我院就诊的疑似深部真菌感染患者93例,均行血清(1, 3)-β-D-葡聚糖检测,同时取下呼吸道分泌物及各种体液标本进行真菌培养,以最终诊断为标准,比较两种检测方法的诊断结果及诊断价值。结果经临床诊断, 93例疑似病例中深部真菌感染35例。血清(1, 3)-β-D-葡聚糖检测深部真菌感染的敏感性明显高于真菌培养,差异具有统计学意义(P <0.05);血清(1, 3)-β-D-葡聚糖检测深部真菌感染的准确性、特异性、阳性预测值及阴性预测值与真菌培养比较,差异均无统计学意义(P>0.05)。结论血清(1, 3)-β-D-葡聚糖检测可有效提高临床诊断深部真菌感染的敏感性,可早期诊断疾病,为治疗方案的制定提供依据。Objective To explore the application value of serum (1, 3)-β-D-glucan in the diagnosis of the deep fungal infection. Methods 93 cases of patients with suspected deep fungal infection in our hospital from September 2014 to March 2018 were selected. The serum (1, 3)-β-D-glucan was detected, and the lower respiratory secretions and various body fluids were taken for fungal culture. With the final diagnosis as the criteria, the diagnostic results and diagnostic value of the two methods were compared. Results Among the 93 suspected cases, 35 cases were diagnosed as deep fungal infection. The sensitivity of serum (1, 3) -β-D-glucan detection in deep fungal infection was significantly higher than that of fungal culture (P 〈0.05); The accuracy, specificity, positive predictive value and negative predictive value in deep ftmgal infection between serum (1, 3) -β-D-glucan and bacteria culture had no statistically significant difference (P 〉0.05). Conclusions The detection of serum (1, 3)-β-D-glucan can effectively improve the sensitivity of the clinical diagnosis of deep fungal infection. It can diagnose the disease early and provide the basis for the formulation of the treatment scheme.
