养阴舒心方对射血分数正常心力衰竭患者生存质量的影响

摘　　要：目的评价养阴舒心方治疗射血分数正常心力衰竭(HFNEF)的有效性及安全性。方法将60例HFNEF患者随机分为治疗组和对照组各30例,治疗组在西药规范治疗的基础上加用养阴舒心方,对照组采用西药规范治疗,两组疗程均为14天。疗程结束后评价明尼苏达心力衰竭生存质量量表(MLHF-Q)评分、中医症状积分、6分钟步行距离(6MWD)、心脏超声相关指标[舒张早期二尖瓣瓣口血流峰值速度/二尖瓣环舒张早期运动速度(E/e')]、血浆脑钠肽(BNP)水平,观察不良反应。结果治疗后两组患者中医症状积分均较治疗前下降(P <0. 05或P <0. 01),治疗组MLHF-Q评分、E/e'水平均较治疗前显著下降(P <0. 05或P <0. 01)。治疗后治疗组MLHF-Q评分、中医症状积分、E/e'均较对照组明显改善(P <0. 05)。两组不良事件发生率比较差异无统计学意义(P> 0. 05)。结论养阴舒心方能够提高HFNEF患者生存质量,改善临床症状,修复心脏舒张功能,安全性较好。Objective To evaluate the efficacy and safety of Yangyin Shuxin Decoction（ 养阴舒心方） in patients with heart failure of normal ejection fraction（ HFNEF）. Methods A total of 60 patients with HFNEF were randomly divided into test group and control group,with 30 patients in each group. Integrated treatment of Yangyin Shuxin Decoction and western medication treatment directed by current guideline for heart failure were applied in the test group for 2 weeks,while only the same western medication was given to the control group. After the treatment,the Minnesota Living with Heart Failure Questionnaire（ MLHF-Q） score and the traditional Chinese medicine（ TCM）symptom score,6-minute walk distance（ 6 WMD）,the cardiac ultrasound related indicators [the transmitral early peak velocity（ E）/early diastolic mitral annulus velocity（ e＇） ] and brain natriuretic peptide（ BNP） were evaluated. The adverse reactions were observed. Results After treatment,the TCM symptom scores of the 2 groups were decreased compared with those before treatment（ P〈 0. 05 or P 〈0. 01）. The score of MLHF-Q score after treatment was significantly improved in the test group compared with the control group（ P〈 0. 05 or P〈 0. 01）. Improvements in TCM symptom scores（ P〈 0. 05） and E/e＇（ P〈 0. 05） were both improved in the test group than those in the control group. There was no significant difference in the incidence of adverse events between groups（ P〈 0. 05）.Conclusion Yangyin Shuxin Decoction could further improve the quality of life,symptoms and the diastolic function in patients with HFNEF on the basis of western medication,which has good safety.
