摘　　要：目的:总结Tubeless VATS技术治疗自发性气胸的经验。方法:回顾性分析应用Tubeless VATS技术治疗自发性气胸6例。结果:本组6例均顺利完成手术,无中转插管,无中转开胸,术中肺萎陷良好,均在单孔胸腔镜下完成手术。手术时间40~80 min,平均57. 5 min;术后苏醒时间5~22 min,平均11. 7 min;术后均无声嘶、呛咳、咽部疼痛、尿潴留;术后4 h、8 h、24 h、48 h疼痛VAS评分均在3级以下;住院时间4~6 d,平均4. 5 d,术后2~5 d出院,平均3. 2 d。出院前及出院1个月后复查胸部CT肺复张良好,无气胸复发。结论:Tubeless VATS技术治疗自发性气胸是安全可靠的,最大限度地减少患者的不适及痛苦,缩短住院日,节约医疗资源,为更多患者提供更好的服务,符合ERAS倡导的理念,值得临床推广及应用。Objective To summarize the Tubeless VATS technical experience in the therapy of spontaneous pneumothoran. Method A retrospective analysis of our hospital, the application of Tubeless VATS technical treatment of 6 cases of spontaneous pneumothorax. Results 6 cases were successfully completed surgery, no transit intubation, without thoracotomy,intraoperative collapsed lung, good are completed under the single - arch thoracoscope surgery. The operation time of 40 - 80 min, an average of 57.5 min ; Postoperative revival time 5 - 22 min, an average of 11.7min. There were no postoperative hoarseness and choking cough, pharyngeal pain, urinary retention ; After 4 hours, 8 hours ,24 hours ,48 hours pain VAS scores were below the level 3 ; Such confinement 4 - 6 clays,an average of 4. 5 days,2 - 5 days out of the hospital,after an average of 3.2 days. Before discharge and discharge after 1 month review chest CT lung after a good, no recurrence of pneumothorax. Conclusion Tubeless VATS treatment of spontaneous pneumothorax technology is safe and reliable, minimize patients discomfort and pain, shorten the confinement, save medical resources and better serve more patients to provide better service,coliform to the ERAS of concept,worthy of clinical popularization and application.
