摘　　要：目的评价临床药师在优化药品不良反应(ADR)监测与报告体系中的作用。方法临床药师参与构建呼吸科ADR管理体系及制度完善;确定每日查房中重点监测的药品,发现疑似ADR及时上报并作统计和分析。结果临床药师参与了呼吸科《药品不良反应监测管理实施细则》的制订并签订了目标责任书;重点监测药品中,呼吸科2017年1月至6月上报的ADR品种数较2016年同期增加4种,增长率为100. 00%; ADR上报例数增加15例,增长率为115. 38%;临床药师上报率为42. 86%,医师漏报率为17. 86%;上报质量较2016年同期提高20. 60%。结论临床药师参与专科化ADR监测与报告体系构建,优化了ADR管理体系,提高了ADR上报率,降低了医师漏报率,也提高了ADR上报质量。在ADR监测与报告体系中,临床药师可发挥核心主体作用。Objective To evaluate the role of clinical pharmacists in optimizing the monitoring and reporting system of adverse drug reaction（ADR）. Methods The clinical pharmacists participated in the construction of ADR management system and the system improve- ment in the Department of Respiratory,determined the key monitored drugs in the daily rounds, found out the suspected ADRs and reported in time,and made the statistical analysis. Results The clinical pharmacists participated in the formulation of ＂ADR Monitoring and Management Implementation Rules＂ and signed the target responsibility document in the Department of Respiratory. Among the key monitored drugs,the sums of ADRs reported from Department of Respiratoryfrom January to June in 2017 increased by 4 kinds com- pared with the same period in 2016,the growth rate was 100.00%.The reported cases of ADRs increased by 15 cases,the growth rate was 115.38%. The reporting rate of clinical pharmacists was 42.86% ,the missing report rate of physicians was 17.86% ,and the quality of report was increased by 20.60% compared with the same period in 2016. Conclusion The participation of clinical pharmacists in the construction of specialized ADR monitoring and reporting system can optimize the management system of ADR,improve the reporting rate of ADR,reduce the missing report rate of physicians and improve the quality of ADR reporting. Therefore,in the ADR monitoring and reporting system,clinical pharmacists can play the core role.
