摘 要:目的探讨糖尿病患者糖化血红蛋白(HbA1c)与肱-踝动脉脉搏波速度(ba PWV)之间的关系,以期为临床预测糖尿病患者发生心脑血管事件的风险提供依据。方法收集2017年1月~2017年12月糖尿病患者152例作为糖尿病组,并根据HbA1c水平将糖尿病组分为低值组、中值组和高值组;同期健康体检者50例作为对照组。记录入组对象身高、体重、TG、TC、HDL-C、LDL-C、HbA1c、FPG等生化指标,采用OMRON BP-203RPEⅢ动脉硬化检测仪检测肱-踝动脉脉搏波速度。结果糖尿病组TC、TG、LDL-C、FPG、HbA1c、ba PWV均高于对照组(P<0.05);糖尿病低、中、高值组3组间TC、HDL-C、LDL-C、FPG、HbA1c、ba PWV比较,差异有统计学意义(P<0.05),高值组TC、TG、LDL-C水平均高于低值组(P<0.05),HDL-C低于低值组(P<0.05);三组间两两比较FPG、HbA1c、ba PWV差异均有统计学意义(P<0.05); FPG、HbA1c与ba PWV呈正相关(P<0.01)。结论糖尿病患者大多伴有脂代谢异常,ba PWV随着HbA1c的增高而增高,提示HbA1c增高可促进动脉硬化的演进,从而增加心血管事件发生的概率。Objective To explore the relationship between glycosylated hemoglobin( HbA1 c) and brachial-ankle pulse wave velocity( baPWV) in patients with diabetes,in order to provide a basis for clinical prediction of the risk of cardiovascular and cerebrovascular events in diabetic patients. Methods 152 cases of diabetes from our hospital's health management Center and cardiovascular medicine Department during January 2017 to December 2017 were collected as the case group( diabetic group) which were divided into low value group,medium value group and high value group according to HbA1c level,and 50 healthy physical examination subjects were selected from the corresponding period as the control group; The height,weight,triglyceride( TG),total cholesterol( TC),high density lipoprotein cholesterol( HDL-C),low density lipoprotein cholesterol( LDL-C),glycosylated hemoglobin( HbA1 c),fasting blood glucose( FPG) and other biochemical indexes of each subject were recorded,while the baPWV of the subjects were tested by OMRON BP-203 RPEIII arteriosclerosis detection device. Results The TC,TG,LDL-C,FPG,HbA1 c and baPWV in the Diabetic group were higher than those of the control group( P〈0.05); The difference of TC,HDL-C,LDLC,FPG,HbA1 c and baPWV among the three groups was statistically significant( P〈0.05). In the high value group,the levels of TC,TG and LDL-C were higher than those in low value group( P〈0.05),while the HDL-C was lower than that in low value group( P〈0.05); furthermore,the difference of FPG,HbA1 c and baPWV were significant between each two groups( P〈0.05); The FPG and HbA1 c were positively correlated with baPWV( P〈0.01). Conclusion Patients with diabetes are mostly associated with abnormal lipid metabolism;baPWV increased with the increase of HbA1 c,indicating that the increase of HbA1 c can enhance the evolution of arteriosclerosis,thereby increasing the probability of cardiovascular events.
