摘  要:目的分析献血者ABO血型鉴定困难的原因,保证血型鉴定的准确。方法收集2012年1月—2015年12月长沙地区221名微板比色法检测的ABO血型正反定型不符的样本,运用经典的试管法进行系统的ABO血型血清学鉴定。结果 221名ABO血型鉴定困难的原因依次为:抗体减弱(51.13%)、存在同种抗体(15.84%)、ABO亚型(14.48%)、其他原因(11.77%)、存在冷凝集素(5.88%)、抗原减弱(0.90%)。共检出32例ABO亚型,其中7例存在不规则抗A或抗B抗体。结论 ABO血型鉴定困难的样本,采用正确的处理方案,可以得到正确的结果,既不浪费血液资源,也可保障输血安全。Objective To analyze the causes of difficulty in identification of ABO blood group of 221 cases of blood donors and ensure the accuracy of blood type identification. Methods The samples of unconformity of positive and reverse ABO blood typing tested by the microplate colorimetric method of 221 cases in the Changsha from Januaty 2012 to December2015 were collected, and the classic test method was used for systematic ABO serological identification of blood group.Results The causes of difficulty in identification of ABO blood group of 221 cases of blood donors: antibody weakened(51.13%), having the same type of antibody (15.84%), ABO subtype (14.48%), other causes (11.77%),having cold agglutinin (5.88%) and antigen weakened (0.90%), the ABO subtype of 32 cases was tested, including 7 cases with irregular antibody A or B. Conclusion The accurate treatment plan for samples with difficulty in identification of ABO blood group can obtain an accurate result, which does not waste the blood resources and ensure the blood transfusion safety.
