摘　　要：目的研究和分析儿童难治性过敏性紫癜与T细胞亚群和食物特异性IgG相关性。方法选取30例难治性过敏性紫癜患儿为观察组和60例一般过敏性紫癜为对照组,分别进行T细胞亚群和食物特异性IgG检测,观察HSP临床表现,并与对照组进行比较,探讨T细胞亚群和食物特异性IgG对儿童难治性HSP的影响,T细胞亚群和食物特异性IgG与儿童难治性HSP及其病情等的关联,并将所得数据进行统计学分析。结果 T细胞亚群中的CD3^+差异无统计学意义,观察组T细胞亚群中的CD4^+和CD4^+/CD8^+较对照组低(P<0.05),CD8^+较对照组高(P<0.05);观察组食物特异性IgG阳性率高于对照组(P<0.05),组间相关差异具有统计学意义。结论对难治性HSP与T细胞亚群和食物特异性IgG相关性研究表明难治性过敏性紫癜患者比一般过敏性紫癜患者的自身免疫能力更差;难治性过敏性紫癜对于更多食物有过敏现象。对难治性HSP与T细胞亚群和食物特异性IgG相关性研究能为临床医生治疗儿童难治性HSP提供指导和方向。Objective To study and analyze the correlation between refractory Henoch-Schonlein purpura and T cell subsets and food specificIgG in children. Methods 30 cases of refractory Henoch-Schonlein purpura were selected as observation group and 60 cases of general Henoch-Schonlein purpura as control group, T cell subsets and food specific IgG were detected, and the clinical manifestations of HSP were observed andcompared with those in control group. Effects of T cell subsets and food specific IgG on refractory HSP in children the relationship between T cellsubsets and food specific IgG and refractory HSP in children and their condition were investigated. Results There was no significant difference inCD3＋ in T cell subsets. The CD4＋ and CD4＋ / CD8＋ in T cell subsets in the observation group were lower than those in the control group （P〈0.05）,and those in the control group were higher than those in the control group （P〈0.05）. The positive rate of food specific IgG in the observation groupwas higher than that in the control group （P〈0.05）. Conclusion The correlation between refractory HSP and T cell subsets and food specific IgGshowed that the autoimmune ability of refractory Henoch-Schonlein purpura patients was worse than that of general anaphylactoid purpura patients.Refractory Henoch-Schonlein purpura is allergic to more foods. The study on the correlation between refractory HSP and T cell subsets and food specificIgG can provide guidance and direction for clinicians in the treatment of refractory HSP in children.
