鼻眶筛骨折开放复位手术的客观疗效分析

摘　　要：目的探讨鼻眶筛骨折的临床特征、手术治疗方法,及Mimics软件在鼻眶筛手术效果客观评价方面的应用。方法选择2012年2月~2016年1月于我科行开放复位手术的鼻眶筛骨折患者,伤后均伴有颌面部畸形,共19例,利用Mimics Research19.0软件对患者术前术后颌面部三维CT进行对比,并进行骨折断端位移测量,进行客观疗效分析。结果患者术后对于面部外形的主观评价满意,且颌面部骨质三维重建示面部外形明显改善,利用Mimics软件进行数据测量,显示术前骨折断端位移是7.29±3.18mm;术后骨折断端位移是0.43±1.07mm。且随访24个月以上,患者对鼻面部外形恢复满意。结论对于伤后出现明显颌面部畸形的鼻眶筛骨折的患者,术后外形得到明显改善,除患者主观评价满意外,应用Mimics软件可以客观地评估鼻眶筛骨折开放复位手术的疗效。Objective To investigate the clinical characteristics and surgical treatment of nasal orbital ethmoid fracture, and the application of Mimics software in the objective evaluation of the effect of nasal orbital ethmoid surgery. Method A total of 19 cases with nasal orbital ethmoid fracture undergoing open reduction surgery from Feb. 2012 to Jan. 2016, all of them were accompanied with maxillofacial malformation. And the preoperative and postoperative three-dimensional CT of maxillofacial were compared by using Mimics Research 19.0 software. The displacement of fracture was measured and the objective curative effect was analyzed. Result The patients were satisfied with the facial appearance after the operation, and the three-dimensional reconstruction of the maxillofacial bone showed a significant improvement in the facial appearance. The data were measured by Mimics software, in which the displacement of the preoperative fracture end was 7.29±3.18mm, and the displacement of the postoperative fracture end was 0.43±1.07mm. After 24 months follow-up, the patients were all satisfied with the appearance. Conclusion For patients with nasal orbital ethmoid fracture accompanied with obvious maxillofacial malformation, the postoperative appearance is obviously improved. In addition to subjective evaluation of patients, Mimics software can objectively evaluate the efficacy of open reduction surgery for nasal orbital ethmoid fracture.
