应用踝关节清理术治疗早中期踝关节骨关节炎的近期疗效观察

摘　　要：目的观察踝关节清理术治疗早期和中期踝关节骨关节炎的近期临床效果。方法回顾分析2015年10月至2017年5月河北医科大学第三医院采用踝关节清理术治疗的22例Ⅱ度、Ⅲ度踝关节骨关节炎患者的临床资料。其中男15例,女7例,年龄(45. 6±7. 5)岁(30～60岁)。22例患者均有踝关节创伤史。采用美国足踝外科协会(AOFAS)踝与后足评分、疼痛视觉模拟评分(VAS)和患者主观满意度分别对术前及术后末次随访踝关节功能进行评价。结果 22例患者均获得随访,随访时间为(22. 0±6. 2)个月(12～31个月)。末次随访时22例患者踝关节疼痛症状均得到显著缓解,踝关节功能均恢复满意,行走步态均正常。术前与末次随访的AOFAS踝与后足评分分别为(52. 6±8. 3)分和(89. 3±7. 0)分,差异有统计学意义(P <0. 05); VAS评分分别为(5. 8±1. 6)分和(1. 0±0. 5)分,差异有统计学意义(P <0. 05);末次随访患者主观满意度优18例、良2例、中1例、差1例,满意率90. 9%。结论应用踝关节清理术治疗早中期踝关节骨关节炎可获得良好的近期疗效。Objective To investigate the short-term clinical effect of ankle joint debridement in the treatment of early and mid-stage ankle osteoarthritis. Methods Twentytwo cases of early and mid-stage ankle osteoarthritis underwent ankle joint debridement fi＇om October 2015 to May 2017 were retrospectively analyzed. There were 15 males and 7 females. The average age of 22 patients at the time of surgery was 45.6 ± 7.5 years （range, 30-60 years）. All patients had a history of ankle trauma. The AOFAS score, VAS score and Evaluation of Patients＇ Subjective Satisfaction were used to evaluate the ankle joint function at preoperation and final follow-up. Results Twentytwo patients were followed up for （ 22.0 ± 6. 2 ） months （ range, 12-31 months）. The pain was significantly relieved at the final follow-up. Satisfactory recovery of ankle function and normal walking gait were achieved. The AOFAS scores and VAS scores for preoperation and final follow-up were 52.6 ± 8.3 and 89.3 ± 7.0 , 5.8 ± 1.6 and 1.0 ± 0.5, respectively. The differences were all statistically significant （all P 〈 0.05 ）. There were 18 excellent, 2 good, one general and one poor in Patient＇s Subjective Satisfaction, and the satisfaction rate was 90.9%. Conclusion The ankle debridement is an effective and convenient treatment for early and mid-stage ankle osteoarthritis.
