米非司酮联合甲氨蝶呤与腹腔镜手术治疗异位妊娠的疗效比较

摘　　要：目的比较米非司酮联合甲氨蝶呤与腹腔镜手术治疗异位妊娠的疗效。方法 96例异位妊娠患者随机分为观察组与对照组各48例。观察组采用腹腔镜手术治疗,对照组采用米非司酮+甲氨蝶呤保守药物治疗。比较2组治疗效果、治疗前后血清绒毛膜促性腺激素(β-HCG)水平、β-HCG恢复正常所需时间及不良反应发生情况。结果观察组痊愈率及总有效率均显著高于观察组(P <0. 05);治疗后,观察组β-HCG水平显著低于对照组,β-HCG恢复正常时间显著短于对照组(P <0. 05); 2组不良反应发生率比较无显著差异(P> 0. 05)。结论米非司酮联合甲氨蝶呤与腹腔镜手术治疗均是异位妊娠的有效治疗方式,但需综合考虑患者具体症状与意愿进行选择。Objective To compare the efficiency of mifepristone combined with methotrexate and laparoscopic surgery in treatment of patients with ectopic pregnancy. Methods A total of 96 patients with ectopic pregnancy were randomly divided into observation group and control group, 48 cases in each group. The observation group was treated with laparoscopic surgery, while the control group was treated with mifepristone and methotrexate. Efficiency, the level of serum chorionic gonadotropin （β-HCG） before and after treatment, the time of β-HCG to normal level and the incidence of adverse reactions were compared between two groups. Results The cure rate and total effective rate of the observation group were significantly higher than those of the control group （ P 〈0.05）. After treatment, the level of β-HCG in the observation group was significantly lower than that of the control group, and the time of β-HCG to normal level was significantly shorter than the control group （ P 〈 0.05）. There was no significant difference in the incidence rate of adverse reaction between the two groups （ P 〉0.05）. Conclusion Both mifepristone combined with methotrexate and laparoscopic surgery are the effective and feasible methods for ectopic pregnancy, but the selection of therapy should be comprehensively considered on the basis of specific symptoms and willing of the patients.
