超声引导下神经阻滞复合喉罩全麻对老年股骨粗隆间骨折患者术中血糖水平的影响

摘　　要：目的探讨超声引导下股神经和股外侧皮神经阻滞复合喉罩全麻对老年股骨粗隆间骨折患者术中血糖水平的影响。方法选取40例单侧股骨粗隆间骨折手术患者,随机分为股神经+股外侧皮神经阻滞复合喉罩全麻组(F组)及单纯喉罩全麻组(L组)。比较2组手术前及手术后1 h的血糖水平。结果与手术前比较,2组手术后1 h血糖水平均显著升高(P <0. 05),且L组血糖水平显著高于F组(P <0. 05)。结论对老年股骨粗隆间骨折手术患者,超声引导下股神经及股外侧皮神经阻滞联合喉罩全麻能有效降低机体应激反应所致的血糖水平升高。Objective To observe effect of ultrasound-guided nerve block combined with general anesthesia of laryngeal mask on blood glucose level in elderly patients with intertrochanteric fracture. Methods A total of 40 patients with unilateral intertrochanteric fracture were randomly divided into two groups. Femoral nerve and lateral femoral cutaneous nerve block combined with general anesthesia of laryngeal mask group （group F） and general anesthesia of simple laryngeal mask group （group L）. Blood glucose levels before and 1 h following surgery in two groups were compared. Results Compared with operation before, the blood glucose level of the two groups increased significantly （ P 〈0.05）, and group L was significantly higher than that of group F after operation （ P 〈 0.05） . Conclusion Femoral nerve and lateral femoral cutaneous nerve block combined with general anesthesia of laryngeal mask can effectively reduce blood glucose level caused by stress response of the body.
