筋脉通防治糖尿病周围神经病变的机制探讨

摘　　要：糖尿病周围神经病变(diabetic peripheral neuropathy,DPN)是糖尿病最常见的并发症之一,其发病机制复杂,西药单一治疗疗效有限。中药复方筋脉通(Jinmaitong,JMT)主要由菟丝子、女贞子、水蛭、元胡、桂枝、细辛等药物组成,具有补肾活血、温通经脉的功效,运用于临床多年,对于DPN有良好的疗效。文章综述历年来课题组有关筋脉通治疗DPN的体内及离体实验研究,探讨其从细胞增殖活性、代谢途径、氧化应激、炎症损伤、自噬及凋亡、神经再生信号通路等途径多靶点的作用机理,以期为JMT治疗DPN和后续研究提供参考依据。Diabetic peripheral neuropathy（ DPN） is one of the most common complications of diabetes with complicated etiology mechanism. The effect of simple western medicine is limited. Compound herbal medicine,Jinmaitong is composed of semen cuscutae,glossy privet fruit,leech,rhizome corydalis,cassia twig,herba asari cum radice,etc.,acting on tonifying kidney,activating blood circulation and warming meridians. This formula has been adopted in clinical practice for years and it achieves the satisfactory effects on DPN. In the paper,the in vivo and vitro experiment researches were reviewed. The effects and mechanism of the formula were explored in terms of the cellular proliferation activity,the metabolic approaches,the oxidative stress,the inflammatory injury,autophagy and apoptosis,as well as the nerve regeneration signal pathway so as to provide the evidences for the treatment of DPN and the follow-up research.
