经皮椎间孔镜下神经根减压松解术治疗腰椎间盘突出症的临床疗效

摘　　要：目的探讨经皮椎间孔镜下神经根减压松解术治疗腰椎间盘突出症的临床效果。方法 25例单节段腰椎间盘突出症患者行经皮椎间孔镜下神经根减压松解术,记录手术切口长度、手术时间、住院费用、术后住院时间,并采用VAS疼痛评分和MacNab评分评价术后临床疗效。结果 25例患者手术切口长度7～11mm,手术时间38～86min,住院费用1.9～3.1万元,术后住院时间3～7d。与术前相比,术后1d、1个月和3个月VAS疼痛评分降低[(2.44±0.77)分、(1.84±0.62)分和(1.08±1.12)分vs.(7.76±1.26)分](P<0.01)。术后3个月MacNab评分为优23例,良2例。结论经皮椎间孔镜下神经根减压松解术能有效改善腰椎间盘突出症患者的临床症状,具有手术创伤小、住院时间短、术后恢复快等优点。Objective To investigate the clinical efficacy of nerve root decompression and release under percutaneous transforaminal endoscopic discectomy（PTED）in the treatment of lumbar disc herniation（LDH）.Methods Twenty-five patients with single-segment LDH accepted nerve root decompression and release under PTED.The length of surgical incision,operation time,hospitalization costs and postoperative hospital stay were recorded.The clinical outcomes were evaluated by VAS pain score and MacNab score.Results The length of surgical incision,operation time,hospitalization costs and postoperative hospital stay of 25 cases were 7-11 mmutes,38-86 minutes,19 000-31 000 yuan and3-7 days,respectively.Compared with before,VAS pain socres on the 1 ^st day,in the 1 ^st month and 3 ^rd month after operation were significantly decreased [（2.44±0.77）points,（1.84±0.62）points and（1.08±1.12）points vs.（7.76±1.26）points]（P〈0.01）.Moreover,MacNab score in the 3^ rd month after operation was excellent in 23 cases and good in 2 cases.Conclusion The nerve root decompression and release under PTED can effectively improve the clinical symptoms of patients with LDH and has the advantages of smaller trauma,shorter hospital stay and quicker recovery.
