摘 要:目的探讨扩血管药物山莨菪碱治疗突发性耳聋的临床效果。方法将我院2017年1月至2018年2月收治的突发性耳聋患者80例随机分为对照组和观察组,每组40例。对照组应用常规药物治疗,观察组在对照组的基础上联合扩血管药物山莨菪碱。比较两组患者耳聋、耳鸣恢复情况。比较两组患者治疗前、后血浆中血栓素A_2(TXA_2)和前列环素I_2(PGI_2)水平、血清白细胞介素6(IL-6)和肿瘤坏死因子α(TNF-α)水平;比较两组患者椎-基底动脉的收缩期峰值流速(PSV)和阻力指数(RI)。结果治疗后,观察组患者耳聋、耳鸣恢复总有效率均高于对照组(P<0.05)。治疗后,两组患者的TXA_2、IL-6及TNF-α水平均降低,PGI_2水平均升高,且观察组优于对照组(P<0.05)。治疗后,观察组患者的PSV水平高于对照组,RI低于对照组(P<0.05)。结论扩血管药物山莨菪碱能够有效提高突发性耳聋患者的治疗有效率,改善耳蜗微循环障碍,改善患者听力。Objective To explore the clinical effect of vasodilator anisodamine on sudden deafness. Methods Eightypatients with sudden deafness admitted in our hospital from January 2017 to February 2018 were randomly divided intocontrol group and observation group, with 40 cases in each group. The control group was treated with conventional medicine,while the observation group was treated with vasodilator anisodamine on the basis of the control group. The status of deafnessand tinnitus resumption were compared between the two groups. The plasma levels of thromboxaneA2 (TXA2) andprostacyclinI2 (PGI2), the serum levels of interleukin6 (IL-6) and tumor necrosis factor α (TNF-α) before and after treatmentwere compared between the two groups. The peak systolic velocity (PSV) and resistance index (RI) of vertebral-basilar arterywere compared between the two groups. Results After treatment, the total effective rates of deafness and tinnitus resumptionin the observation group were higher than those in the control group (P〈0.05). After treatment, the levels of TXA2, IL-6 andTNF-α in both groups decreased, and the level of PGI2 increased, and those of the observation group were superior to thecontrol group (P〈0.05). After treatment, the level of PSV in the observation group was higher than that in the control group,while the RI was lower than that in the control group (P〈0.05). Conclusion Vasodilator anisodamine can effectively increasethe treatment efficacy of sudden deafness, improve cochlear microcirculation disorders and hearing.
