隔姜灸神阙穴联合参苓白术散对艾滋病脾虚证腹泻及免疫功能的影响

摘　　要：目的:探讨隔姜灸神阙穴联合参苓白术散治疗艾滋病脾虚证腹泻的临床疗效及对免疫功能的影响。方法:选择2015年6月至2017年1月重庆市公共卫生医疗救治中心收治的艾滋病合并腹泻且属脾虚证的患者93例作为研究对象,根据随机数表法分为观察组和对照组。观察组采用隔姜灸神阙穴与参苓白术散治疗,对照组采用口服黄连素与补液治疗。分析比较2组患者的临床疗效、免疫功能和生存质量。结果:治疗后观察组中医临床评分明显低于对照组(P<0.05),总有效率84.91%高于对照组的67.50%(P<0.05)。治疗后观察组CD_4^+T细胞、Th17细胞含量均高于对照组,CD_8^+T细胞、Treg细胞含量均低于对照组(P<0. 05)。观察组的情绪功能、疼痛、精力、心理功能因子评分均高于对照组(P<0. 05)。结论:隔姜灸联合参苓白术散治疗艾滋病脾虚证腹泻患者疗效较好,且对改善免疫功能具有积极作用。Objective： To investigate the clinical effect of ginger-partitioned moxibustion at Shenque point combined with shenlingbaizhu powder on diarrhea due to spleen deficiency syndrome of AIDS and its influence on immune function. Methods： Ninety-three cases of patients with AIDS diarrhea due to spleen deficiency syndrome were selected as research objects from June 2015 to January 2017 in Chongqing Public Health Medical Treatment Center, and were randomly divided into the observation group and the control group The observation group was treated with moxibustion insulated by ginger at Shenque acupoint combined with shenlingbaizhu powder, and the control group was treated with berberine orally and fluid replacement. The clinical efficacy, immune function, and life quality of the two groups was analyzed and compared. Results： After treatment, clinical score of Traditional Chinese Medicine in the observation group was significantly lower than that in the control group （P 〈0.05）, and the total effective rate of the observation group （84. 91% ） was significantly higher than that in the control group （67. 50% ）, P 〈0.05. After treatment, the content of CD4＋T cells and Th17 cells in the observation group was higher than those in the control group （P 〈0. 05）, and the content of CD8 ＋T cell and Treg cell in the observation group was lower than those in the control group （P 〈0. 05） . The MOS-HIV scores of emotional function, pain, energy, and psychological function factors in the observation group were higher than those in the control group （P 〈 0. 06） . Conclusion： The therapeutic effect of moxibustion insulated by ginger at Shenque acupoint combined with shenlingbaizhu powder on the treatment of AIDS diarrhea due to spleen deficiency syndrome was better, and could improved immune function.
