摘　　要：目的探讨WILLIS覆膜支架治疗颅内复杂动脉瘤的初步经验。方法搜集采用WILLIS覆膜支架治疗颅内复杂动脉瘤患者6例(其中血泡样动脉瘤2例,巨大动脉瘤3例,垂体瘤手术致假性动脉瘤1例,共6个动脉瘤),分析患者的临床资料和疗效。结果 6例患者共置入WILLIS覆膜支架6枚,均一次成功置入支架;4例一次球扩后支架贴壁良好封堵瘤颈,动脉瘤不显影,1例3次球扩后,造影动脉瘤不显影,1例4次球扩后造影动脉瘤仍淡淡显影,10 min后造影动脉瘤不显影。4例术后3个月血管造影随访,WILLIS覆膜支架治疗动脉瘤均无复发,载瘤动脉通畅,所有患者随访无再次蛛网膜下腔出血及血栓栓塞症状,预后良好。结论 WILLIS覆膜支架治疗颅内复杂动脉瘤疗效好,无需瘤腔内弹簧圈栓塞,但需严格掌握适应症,术中对动脉瘤血液动力学影响小及术后复发率低是其突出的优点,但仍需更长期的随访和大样本多中心研究。Objective To investigate preliminary experience of WILLIS covered stent therapy intracranial complex aneurysm. Methods Adopt WILLIS covered stent therapy intracranial complex aneurysm 6 cases from January 2015 to April2015( Among blood blister like aneurysms 2 cases,giant aneurysms 3 cases. Pituitary adenoma surgery false aneurysm 1case. Results A total of 6 patients were implanted with 6 WILLIS covered stent. All cases were homogeneous successfully implanted stent. One time ball expanded stent stick wall well blocking aneurysm neck in 4 cases. Aneurysm not shows shadow,Aneurysm not show shadow three times ball expanded in 1 case,Aneurysm shows light shadow four times ball expanded in 1 case,Aneurysm not shows shadow after 10 minutes. 4 cases DSA were followed up for 3 months after operation; WILLIS covered stent therapy intracranial not recurrence and Load Aneurysm artery patency. All patients were followed up without subarachnoid hemorrhage and thrombosis,the prognosis is good. Conclusion WILLIS covered stent therapy intracranial complex aneurysm have good effect. No aneurysm cavity coil embolization. But it is necessary to strictly control the indications. Influence hemodynamic of the aneurysms small during the operation and low recurrence rate after surgery is a prominent advantage. But still need to be more long term follows up and large sample and multi center study.
