摘　　要：探讨克氏针张力带钢丝和锁骨钩钢板2种不同固定物治疗锁骨远端骨折临床效果。方法：招募2016年5月～2017年5月就诊的30例锁骨远端骨折患者分析其临床资料，对照组15例患者采用克氏针张力带钢丝内固定，观察组15例患者采用锁骨钩钢板内固定。比较2组手术时间、住院时间、失血量及手术1次成功率，JOA评分评估患者肩部恢复。结果：2组手术时间、失血量相比，差异无显著性（P〉0．05）。与对照组相比，观察组住院时间显著降低（P〈0．01），且手术1次成功率为96．67％。与对照相比，观察组JOA评分中疼痛、功能、活动度、X线评价4个方面的评分分别显著升高，差异具有统计学意义（P〈0．01），而关节稳定性方面无差异。此外，2组总评分相比，差异也同样显著（P〈0．01）。结论：与克氏针张力带钢丝内固定相比，锁骨钩钢板作为安全有效的内固定物，在治疗锁骨远端骨折中固定可靠、住院时间短，肩部功能恢复快。To compare the effect of kirschner wire tension band and clavicular hook plate in the treatment of distal clavicle fracture. Methods： Retrospective analysis of our hospital in May 2016 to May 2017 30 cases of distal clavicle fracture in patients with clinical data, which would use kirschner wire tension wire fixation in 15 eases into the control group, the use of clavicle 15 cases of inter- nal fixation of hook plate were included in the study group. The success rate of operation time, hospitalization time, blood loss and opera- tion were compared between the two groups. The recovery of shoulder function was evaluated by JOA score. Results： There was no signifi- cant difference between the two groups in operation time and blood loss （ P 〉 0.05）. Compared with the control group, the hospital stay was significantly lower （ P 〈 0.01 ） , and the success rate was 96.67%. Compared with the control group, the scores of pain, function, activity and x - ray evaluation in the JOA score of the study group were significantly higher than those of the control group （ P 〈 O. 01 ）. There was no significant difference in joint stability. In addition, the differences were also significant （ P 〈 0.01 ）. Conclusion： The cla- vicular hook plate was a kind of safe and effective fixation for the distal fracture of the clavicle with fixed fixation, short hospital stay and fast recovery of shoulder function compared with Kirschner wire tension fixation.
