摘 要:目的探究低剂量64层螺旋CT直接静脉造影(MSCTV)在诊断盆腔、下肢静脉血栓上的应用效果。方法对复旦大学附属中山医院青浦分院收治的80例疑似盆腔、下肢静脉血栓患者行低剂量64层螺旋CT直接静脉造影,并与数字减影血管造影(DSA)结果进行比较。结果经MSCTV检查,共有61例存在不同部位、不同程度的盆腔、下肢静脉血栓、部位和范围清晰明确。MSCTV检查在不同部位的图像质量评分明显优于DSA检查,差异有统计学意义(P<0.05)。MSCTV在诊断盆腔及下肢静脉血栓上的敏感度、特异度以及阳性、阴性预测值分别为95.31%、100.00%、100.00%和84.21%。结论低剂量64层螺旋CT直接静脉造影能够清晰显示盆腔、下肢静脉血栓情况以及病变血管周围的解剖结构,具有成像清晰、快速准确和无创伤性等优势。OBjECTIVE To research the application of low dose 64 slice spiral CT(MSCTV)direct venography in pelvic and lower extremity venous thrombosis. METHODS 80 patients with suspected pelvic and lower extremity venous thrombosis were treated with low dose 64 slice spiral CT direct venography, and compared with the results of DSA.RESULTS According to MSCTV examination, there were 61 cases of venous thrombosis in different parts and degrees of pelvic and lower extremities. The location and scope of thrombus were clear and definite. The image quality scores of the MSCTV examination in different parts were obviously better than those of the DSA examination, and the differences were statistically significant(P〈0.05). With DSA as the gold standard, the sensitivity, specificity, positive predictive value and negative predictive value of MSCTV in the diagnosis of pelvic and lower venous thrombosis were 95.31%, 100%, 100% and 84.21%, respectively. CONCLUSION The low dose 64 slice spiral CT direct venography could clearly show the venous thrombosis of pelvic and lower extremities and the anatomical structure around the vessels of the lesion. It had the advantages of clear, fast and accurate imaging, no traumatic and so on.
