某院ICU患者质子泵抑制剂使用的合理性分析

摘　　要：目的调查某医院ICU质子泵抑制剂(PPIs)的使用情况,分析其使用的合理性,为PPIs的合理使用提供参考依据。方法查阅该院2016年7月～2017年6月ICU科室使用过质子泵抑制剂的患者病历,排除在ICU使用质子泵抑制剂少于2d的患者病历,对外科、内科ICU患者使用质子泵抑制剂的品种、用法用量、给药途径、用药疗程和用药指征等进行分析。结果共调查了1 293例患者,其中外科ICU患者567例,内科726例,ICU患者使用PPIs的主要原因是应激性溃疡的预防用药,外科患者以手术后预防应激性溃疡为主,适应证72%(931/1 293)的病例使用合理,普遍存在用药疗程及偏爱使用注射剂型等问题。结论该院质子泵抑制剂在ICU患者中使用广泛,存在无指征用药、长时间用药等不合理现象,需加强完善质子泵抑制剂的临床应用。Objective To investigate the use of ICU proton pump inhibitor （PPIs） in a hospital,to analyze the rationality of its use,and to provide a reference for the rational use of PPIs. Methods From July 2016 to June 2017,the medical records of patients who had used proton pump inhibitors in the ICU were reviewed.The patients who were treated with PPIs in ICU for less than 2 d were excluded.The variety,dosage,way of administration,course of treatment,indication of drug use,etc were analyzed. Results A total of 1 293 cases were investigated,including 567 cases of surgical ICU and 726 cases of internal medicine ICU.The main reason for the use of PPIs was the prevention of stress ulcer,and the main reason for the use of surgical ICU was to prevent stress ulcer after operation.The indications of 72% （931/1 293） patients were not reasonable,and there were some problems such as course of treatment and the preference for injection type. Conclusion PPIs were widely used in ICU patients and there are unreasonable phenomena such as no indication and long term use.It is necessary to improve the clinical application of PPIs.
