自体心包片修补术干预先天性室间隔缺损临床疗效及心功能改变

摘　　要：目的:探讨自体心包片修补术干预先天性室间隔缺损临床疗效、心脏功能情况及不良反应情况。方法:选择2016-01～2017-12间我院收治的先天性室间隔缺损患儿114例,按病人就诊时间排序,奇偶数法均分为观察组(57例)行自体心包片修补术和对照组(57例)行涤纶布补片修补术,比较两组干预先天性室间隔缺损的临床疗效及对心脏功能的影响及术后不良反应和并发症的发生率。结果:两组病人术后左心射血分数等心功能指标显著改善(P<0.05),但两组间心功能差异无显著性;比较两组手术成功率差异有统计学意义(P=0.022);观察组(3.51%)术后血红蛋白尿较对照组(15.79%)差异有统计学意义(P=0.026),两组残余分流发生率无明显差异(P>0.05);观察组(12.28%)明显低于对照组(33.33%)不良反应总发生率(P=0.007),其中两组血栓及栓塞的发生差异明显(P=0.047)。结论:自体心包片修补术干预先天性室间隔缺损,与涤纶布修补相比,不良反应更少,排斥反应更弱,可作为主要修补手术应用于先天性心脏病的治疗。Objective： To investigate the clinical efficacy, cardiac function, and adverse reactions of self-concentric pericardial patch repair in patients with prosthetic ventricular septal defect. Methods： A total of 114 children with congenital ventricular septal defect who were admitted to our hospital between January 2016 and December 2017 were selected and ranked according to the patient′s visit time. The odd and even number methods were divided into observation group （57 cases） with autologous pericardial patch repair. The clinical and control groups （57 cases） were treated with patch repair of polyester cloth. The clinical efficacy of the two groups of stem ventricular septal defect and the effect on cardiac function and the incidence of postoperative adverse reactions and complications were compared. Results： There was a significant difference in cardiac function parameters between the two groups at 1month after operation （ P 〈 0.05 ）. But compared with observation group ,there was no significant difference（ P 〉 0.05 ）, The success rates between the two groups were significant （ P = 0.022 ） . There was a significant difference in the hemoglobinuria between the observation group （ 3.51 %） and the control group （ 15.79 %）（ P = 0.026 ）. There was no significant difference in the incidence of residual shunt between the two groups （ P 〉 0.05 ）. Compared with the overall incidence of adverse reactions in control group （ 33.33 %）, the observation group （ 12.28 %） was obviously lower （ P = 0.007 ）, and significant differences in thrombosis and embolism between the two groups （ P = 0.047 ）. Conclusion： Autologous pericardial patch repair of congenital ventricular septal defect, compared with polyester cloth repair, fewer adverse reactions, rejection is weaker, can be used as the main repair surgery for the treatment of congenital heart disease.
