摘  要:目的:分析MRI多序列成像技术在诊断及鉴别乳腺癌中的应用价值。方法:选取2017-04~2018-04期间我院收治的乳腺肿瘤病人96例为研究对象,均在入院后进行1.5 T MRI多序列成像[T1加权成像(T1WI)、抑脂T2加权成像(T2WI)、扩散加权成像(DWI)、动态增强成像(DCE)、氢质子波谱成像(1 H-MRS)]扫描,以手术病理为金标准,分析乳腺癌(恶性组)、乳腺良性肿瘤(良性组)病人MRI下各序列成像特点,评估MRI各序列成像技术对乳腺癌的诊断效能。结果:本研究中病理证实乳腺癌58例,乳腺良性肿瘤38例,恶性组MRI征象中边缘不规则、边缘强化比例高于良性组(P<0.05),两组形状、T2WI表现比较差异无统计学意义(P>0.05);在DWI序列上,b值=800 s/mm2时,恶性组ADC值低于良性组(P<0.05),在DCE序列上恶性组TIC曲线与良性组比较差异有统计学意义(P<0.05),恶性组以流出型为主,良性组以流入型为主(P<0.05);多序列成像技术中,以DCE诊断乳腺癌的灵敏度、特异度、准确度最高,而各序列综合诊断时效能更好。结论:MRI多序列成像技术在乳腺癌诊断及鉴别诊断中有一定价值,可较好区分良恶性病变,其中以DCE序列诊断效能最高,而各序列综合诊断价值更高。Objective: To analyze the application value of MRI multi-sequence imaging techniques in the diagnosis and differentiation of breast cancer. Methods: 96 cases of breast cancer patients treated in our hospital from April 2017 to April 2018 were selected for the study, and were given 1.5 T MRI multi-sequence imaging scans [T1-weighted imaging (T1WI) or fat-suppressed T2-weighted imaging (T2WI), diffusion-weighted imaging (DWI), dynamic contrast-enhanced imaging (DCE), and hydrogen proton MR spectroscopy (1 H-MRS)] after admission. Surgical pathology was used as the gold standard to analyze the imaging features of each sequence under MRI of patients with breast cancer (malignant group) and benign breast tumor (benign group). The diagnostic efficacy of each MRI sequence imaging technique for breast cancer was evaluated. Results: 58 cases of breast cancer and 38 cases of benign breast tumor were confirmed by pathology in this study, and the proportions of irregular margin and marginal enhancement in malignant group were higher than those in benign group ( P 〈 0.05 ). There was no significant difference in shape and T2WI manifestation between the two groups ( P 〉 0.05 ). In the DWI sequence, when b=800 s/mm^2, the ADC value in malignant group was lower than that in benign group ( P 〈 0.05 ). There was a statistical difference in the TIC curve between malignant group and benign group in the DCE sequence ( P 〈 0.05 ), and malignant group was dominated by efflux type, and benign group was mainly inflow type ( P 〈 0.05 ). In multi-sequence imaging technique, the sensitivity, specificity and accuracy of DCE were the highest in the diagnosis of breast cancer, and the synthetic diagnosis of each sequence had better diagnostic efficacy. Conclusion: MRI multi-sequence imaging techniques have certain value in the diagnosis and differential diagnosis of breast cancer. And they can distinguish between benign and malignant lesions.
