单唾液酸四己糖神经节苷脂治疗急性脑梗死的有效性与安全性的Meta分析

摘　　要：目的:系统评价单唾液酸四己糖神经节苷脂的有效性与安全性。方法:计算机检索PubMed/Medline、Cochrane library、万方数据库、维普数据库和中国学术期刊全文数据库,全面收集单唾液酸四己糖神经节苷脂治疗急性脑梗死的随机对照临床试验(RCT);利用Cochrane评价标准进行文献质量评价,使用RevMan 5. 3软件进行Meta分析。结果:纳入15篇文献,共计1 667例患者。Meta分析结果显示,单唾液酸四己糖神经节苷脂(GM1)组的有效率和日常生活能力评分(Barthel指数)显著高于常规治疗组(P <0. 001);试验组神经功能缺损的改善情况(NIHSS和CSS)均显著优于对照组(P <0. 001)。采用敏感性分析验证了结果的稳健性。纳入的所有研究中,试验组仅出现3例不良反应。结论:在常规治疗的基础上加用单唾液酸四己糖神经节苷脂治疗急性脑梗死有较好的有效性和安全性,但仍需高质量、大样本的RCT进一步验证其有效性和安全性。Objective： To systematically assess the effectiveness and safety of monosialoteterahexosyl ganglioside. Methods： Randomized controlled trials（ RCTs） which compared monosialoteterahexosyl ganglioside with acute ischemic stroke were collected by searching PubMed/Medline,Cochrane library,WanFang database,CQVIP,CNKI. Meta-analysis was conducted based on the methods recommended by RevMan 5. 3 of the Cochrane Collaboration. Results： 15 randomized controlled trials were included,involving 1 667 patients. System evaluation results show,compared with the conventional treatment group,total effective rate and Barthel index of GM1 were significantly higher（ P〈0. 001）,NIHSS and CSS were significantly lower（ P〈0. 001）. Only 3 adverse reactions were found in the experimental group. There were no differences in adverse reactions between two groups. Conclusion：Monosialoteterahexosyl ganglioside with conventional treatment performed better efficacy and safety for acute ischemic stroke. However,more large-scale and high-quality RCTs are still needed to prove the efficacy and safety of monosialoteterahexosyl ganglioside on acute ischemic stroke.
