小剂量泼尼松联合甲氨蝶呤治疗环状弹力纤维溶解性巨细胞肉芽肿1例

摘　　要：患者男,47岁。肢体曝光部位多发红棕色丘疹、斑块2周。部分皮损融合呈多环状似蕾丝样。组织病理检查见真皮网状层为主大量淋巴细胞、组织细胞弥漫性浸润,部分组织细胞融合形成多核巨细胞。van Gieson染色可见真皮炎性浸润区域弹力纤维完全消失,Alcian blue染色阴性。皮损经小剂量泼尼松联合甲氨蝶呤治疗1个月后大部分消退。A 47-year-old male presented with multiple brownish-red papules and plaques for 2 weeks involving the sun-exposed areas. Some lesions anastomosed into a lace-like multi-annular pattern. Pathology showed diffused mixed inflammatory infiltrate with lymphocytes and histiocytes in the reticular dermis, and some histiocytes fused into multinucleate giant cells. Van Gieson staining revealed complete loss of elastic fibers in the inflammatory infiltrate areas. Alcian blue staining yielded negative result. The lesions were in remission after the treatment of low dose prednisone plus methotrexate regimen within one month.
