异维A酸胶囊不同给药方法治疗中重度痤疮临床观察

摘　　要：目的观察异维A酸胶囊不同给药方法治疗中重度痤疮的疗效,并评价其安全性。方法 45例中重度痤疮患者随机分为研究组23例和对照组22例。研究组采用间歇疗法给予异维A酸胶囊;对照组采用传统每日疗法给予异维A酸胶囊。4周为1疗程。分别在4周、8周、12周、16周观察临床疗效及副作用。结果两组不同给药方法中,研究组中患者在4周时有效率低于对照组(P <0. 05);随着用药时间延长,8周、12周、16周两组间疗效差异逐渐减少,无统计学意义(P均>0. 05)。研究组药物不良反应指标发生率明显低于对照组(P均<0. 05)。结论随着服药时间的延长和累积服药剂量的增加,间歇疗法和每日疗法的疗效相当,但间歇疗法的副作用在整个疗程中较少。在临床上推荐使用间歇疗法口服异维A酸胶囊治疗中重度痤疮。Objective To observe the effect of different approaches of isotretinoin capsules for the treatment of moderate-severe ache and to evaluate the safety. Methods Fortyfive patients with moderate-severe ache were randomized into study group （n = 23 ） and control group （n = 22）. The study group were treated with intermittent therapy to give isotretinoin capsules. The control group were treated with traditional daily therapy to give isotretinoin capsules. Four weeks are a course of treatment. Clinical efficacy and side effects were observed at 4week,Sweek, 12week and 16week respectively. Results In the two groups, the total effective rate of the patients in the study group was lower than that of the control group at 4 weeks（P 〈 0. 05）. With the extension of drug use, the difference of curative effect between the two groups was reduced gradually in 8,12,and 16 weeks,and there was no difference between the two groups（P 〉 0. 05 ）. The clinical adverse drug reaction rate of the study group was significantly lower than that in the control group （ P 〈 0. 05 ）. Conclusion As the duration of medication increases and the cumulative dose increases, the curative of intermittent therapy and daily therapy is comparable. The side effects of intermittent therapy are less throughout the course of treatment. In the clinical practice, isotretinoin capsule intermittent therapy is recommended for moderate-severe acne care.
