国医大师洪广祥教授应用气机升降理论辨治肺系病症思想探讨

摘　　要：国医大师洪广祥教授诊治肺系疾病经验丰富,文章重在总结其临床应用气机升降理论学术特色。主要内容包括三大方面:(1)洪广祥教授的气机升降思想,洪老认为:肺失肃降、气机上逆是肺系病症的根本病理变化;肝升肺降为人体气机升降之枢纽;补脾益肾助肺宣发;胃降腑通协肺肃降。(2)气机升降与肺系病症,包括:气阳虚弱是慢性咳喘病的主要内因,痰瘀是慢性咳喘病最主要的病理产物,宗气虚衰是肺胀发生、发展的中心环节,气机逆乱是慢性干咳的关键病机。(3)气机升降与组方用药,体现在:补肺益气固卫防复发;舒肝泻肺以涤痰行瘀治标实;升阳举陷疗慢性阻塞性肺疾病;调气降逆平干咳。HONG Guang-xiang, a great master of Chinese medicine, has rich experience in the diagnosis and treatment of pulmonary diseases. This paper mainly summarizes the academic characteristics of his clinical application of descending and ascending of qi activities theory. The main content includes three aspects：（1） Professor HONG Guang-xiang＇s thoughts of descending and ascending of qi activities. He believes that the fundamental pathological changes of pulmonary diseases are pulmonary failure of descension and upward inversion of qi. Liver ascending and lung descending is the pivot of descending and ascending of qi activities of the body. Invigorating spleen and kidney for benefiting lung governing diffusion; Lowering stomach and purging fu organs for benefiting lung governing purification and descending.（2） Relationship between descending and ascending of qi activities and pulmonary disease, it includes 4 aspects： deficiency of qi and yang is the main internal cause of chronic cough and asthma, phlegm and blood stasis are the main pathological products of chronic cough and asthma, deficiency of pectoral qi is the central link of the occurrence and development of pulmonary distension, qi-movement disturbance is the key pathogenesis of chronic dry cough.（3） Ascending and descending of qi activities and composition of prescription, are embodied in： invigorating lung qi and strengthening defensive qi to prevent recurrence, smoothing the liver and purging lung in order to resolving phlegm and blood-stasis in treatment of evil sthenic, ascending up spleen-qi and yang in treatment of COPD, regulating qi and falling the versus in treatment of dry cough.
