芩楼清利汤治疗亚急性湿疹湿热证的临床研究

摘　　要：目的:观察芩楼清利汤治疗亚急性湿疹湿热证的临床疗效、安全性研究及可能的作用机制探讨。方法:采取随机对照的研究方法,将纳入的亚急性湿热证患者随机分为治疗组和对照组,治疗组口服芩楼清利汤,对照组口服盐酸西替利嗪片,两组患者均外用复方苦参止痒软膏。观察治疗前后两组皮损形态、皮损面积、瘙痒程度及对血常规及血肌酐、血尿素氮、丙氨酸氨基转移酶、天门冬氨酸氨基转移酶的影响。同时抽取两组患者治疗前及治疗后血清,检测IL-4水平和健康组比较。结果:治疗组愈显率及总效率有显著高于对照组(P<0.05)。两组治疗后瘙痒程度评分均较治疗前明显降低(P<0.05)。两组治疗后皮损面积评分均较治疗前明显降低(P<0.05),治疗组降低明显大于对照组(P<0.05)。两组治疗前IL-4水平均比健康组较治疗前明显升高(P<0.05)。经治疗两组IL-4水平均明显降低(P<0.05)。结论:芩楼清利汤治疗亚急性湿疹湿热证有较好疗效,且安全性满意,可能的作用机制之一是通过下调血清IL-4水平来发挥作用。Objective： To observe the clinical efficacy and safety of Qinlou Qingli Decoction in treating subacute eczema damp-heat type and explore its possible mechanism. Methods： A randomized controlled study was adopted. The patients with subacute damp-heat type were randomly divided into treatment group and control group. The treatment group was taken Qinlou Qingli Decoction and the control group was orally administered with Cetirizine Hydrochloride Tablets, and the two groups were all treated with Compound Kushen antipruritic ointment. Appearance and area of skin lesions, symptoms of itching and safety index including ALT, blood BUN and Cr, blood and urine routine were observed pre and post the treatment. The serum levels of IL-4 in two groups before and after treatment were compared with those in healthy group. Results： There were significant differences in total effective, curative-obvious-effective rate, area of skin lesions between groups（P0.05）. The levels of IL-4 in the two groups were significantly higher than healthy group before treatment, and decreased after treatment（P0.05）. Conclusion： Qinlou Qingli Decoction is effective and safe in the treatment of damp-heat type of subacute eczema. One of the possible mechanisms is to play a role by downregulating serum IL-4 level.
