摘 要:目的探讨等速肌力训练改善腰段不完全脊髓损伤患者下肢肌力与步行能力的效果。方法选取2016年3月—2017年3月该院收治的84例腰段不完全脊髓损伤患者作为研究对象,根据随机数字表将患者分为二组(每组42例)。对照组给予常规康复训练,观察组给予等速肌力训练结合常规康复训练,二组均训练3个月。比较二组下肢肌力、步行能力和独立性。结果二组治疗后下肢肌力评定指标均明显优于治疗前(P<0.05),观察组治疗后腘绳肌峰力矩体重比、股四头肌峰力矩体重比、腘绳肌峰力矩、股四头肌峰力矩评分均较对照组明显改善(P<0.05);二组治疗后脊髓损伤步行指数、6min步行测试、10m步行时间测试、脊髓独立性评定评分均明显优于治疗前(P<0.05),观察组治疗后脊髓损伤步行指数、6min步行测试、10m步行时间测试、脊髓独立性评定评分均明显优于对照组(P<0.05)。结论等速肌力训练结合常规康复训练应用于腰段不完全脊髓损伤患者的康复治疗中,可明显改善患者的下肢肌力、步行能力和功能独立性。Objective To investigate the effect of isokinetic training on lower limb muscle strength and walking ability in the patients with incomplete lumbar spinal cord injury.Methods Totally 84 patients with incomplete lumbar spinal cord injury treated in author′s hospital from March 2016 to March 2017 were selected and equally divided into two groups.The control group was given routine rehabilitation training,while the study group received the isokinetic training based on control,both tid for 3 mon.The muscle strength,walking ability and independence of the lower limbs were compared between the two groups.Results The evaluation index of lower limb muscle strength after treatment in both groups was significantly improved(P〈0.05).The scores of the peak torque to body weight ratio of hamstring(H-PT/BW),PT/BW of quadriceps femoris(QFPT/BW),H-PT and QF-PF in the study group were better than those of the control group(P〈0.05);and the scores of the walking index for spinal cord injury(WISCI Ⅱ),6-minute walking test(6 MWT),10 MWT and spinal cord independence measure(SCIM Ⅲ)of the two groups were significantly increased,and were better in the study group than in the control group(P〈0.05).Conclusion The application of the isokinetic training combined with routine rehabilitation training can significantly improve the muscle strength,walking ability and functional independence of the lower extremities for patients with incomplete lumbar spinal cord injury.
