摘 要:[目的]寻找阴虚上火的新型诊断标志物,深化对阴虚上火的认识。[方法]对34例受试者血清样本的临床资料进行统计分析,运用ELISA法检测血清单核细胞分化抗原(monocyte differentiation antigen)CD14、激肽原-1(Kininogen-1,KNG-1)、载脂蛋白B(apolipoprotein B-100,APOB)和载脂蛋白A2(apolipoprotein A2,APOA2)的浓度,并运用ROC曲线对上述蛋白诊断上火的效果进行评估。[结果]阴虚上火患者CD14与KNG-1显著上调,APOB与APOA2显著下调,与健康对照组比较,差异具有统计学意义(P<0.001,P<0.05,P<0.05,P<0.01)。将CD14、APOA2、KNG-1进行组合构建诊断模型,ROC曲线显示组合物的曲线下面积(area under curve,AUC)值为0.934,敏感度和特异度分别为100%和71.40%,具有较好的准确性。[结论]阴虚上火与能量代谢、炎症反应密切相关。CD14、APOA2、KNG-1的组合可作为临床诊断阴虚上火的潜在的生物学标志物,有助于中医疾病诊断的客观化。[Objective]To screen new diagnostic biomarkers of Yin-deficiency-heat(YDH) syndrome and to deepen the understanding of it. [Methods]The clinical data of 34 human serum samples including 17 YDH syndrome patients and 17 healthy controls were statistically analyzed in this study. The serum specific protein concentration was detected by ELISA and the diagnostic effect of proteins were evaluated by ROC curve. [Results]We found that monocyte differentiation antigen CD14 and Kininogen-1(KNG-1) were significantly increased(P〈0.001,P〈0.05), and apolipoprotein B-100(APOB) and apolipoprotein A2(APOA2) were significantly decreased in YDH syndrome patients(P〈0.05,P〈0.01), suggesting that YDH syndrome may cause excessive energy consumption and inflammation. In addition, CD14, APOA2 and KNG-1 were combined to assess the diagnostic effect of YDH syndrome. The ROC curve suggested that the AUC value was 0.934, the sensitivity and specificity were 100% and 71.40%, respectively. The combination of three proteins had good accuracy and could be used as potential biological biomarkers for YDH syndrome diagnosis. [Conclusion]YDH syndrome is closely related to energy metabolism and inflammatory reaction. The combination of CD14, APOA2, and KNG-1 provides potential biomarkers for the clinical diagnosis of YDH syndrome and contributes to the objective diagnosis of TCM disease.
