经皮穴位电刺激辅助静脉全麻对妇科宫腔镜手术患者镇痛效果的影响

摘　　要：[目的]探讨经皮穴位电刺激辅助静脉全麻对妇科宫腔镜手术患者镇痛效果的影响。[方法]选取2017年3月至2018年3月本院收治的妇科宫腔镜手术患者100例,依据随机数字表法分为电麻组和全麻组,每组50例,全麻组给予静脉全麻,电麻组在此基础上给予经皮穴位电刺激辅助麻醉,比较两组血流动力学、镇痛效果、麻醉药物用量、不良反应。[结果]电麻组麻醉后10min(T1)、刮宫操作(T2)、术毕(T3)的平均动脉压(mean arterial pressure, MAP)、心率(heart rate, HR)明显低于全麻组,差异有统计学意义(P<0.05);电麻组麻醉优良率明显高于全麻组,差异有统计学意义(P<0.05);电麻组T3疼痛视觉模拟评分法(VAS)得分明显低于全麻组,差异有统计学意义(P<0.05);电麻组麻醉药物用量、不良反应发生率明显低于全麻组,差异有统计学意义(P<0.05)。[结论]经皮穴位电刺激辅助静脉全麻可有效改善妇科宫腔镜手术患者血流动力学,有利于提高患者镇痛效果,且可有效减少麻醉药物用量、不良反应,值得临床作进一步推广。[Objective] To discuss the effect of transcutaneous electrical acupoint stimulation and intravenous general anesthesia on the analgesic effect in patients undergoing gynecologic hysteroscopy. [Methods] 100 patients undergoing gynecologic hysteroscopy were selected from March 2017 to March 2018 in our Hospital, according to the random digital table method, they were divided into the electric anesthesia group and the general anesthesia group, 50 cases in each group, the general anesthesia group was given intravenous general anesthesia, the electric anesthesia group on the basis was given the transcutaneous electrical acupoint stimulation. The hemodynamics, analgesic effect, narcotic drugs dosage and adverse reaction were compared in the two groups. [Results] The mean arterial pressure（MAP）, heart rate（HR） of after anesthesia 10 min（T1）, curettage operation（T2）, end of operation（T3） in electric anesthesia group were significantly lower than those in general anesthesia group, the difference was statistically significant（ P〈0.05）. The anesthesia excellent rate in electric anesthesia group was significantly higher than that in general anesthesia group, the difference was statistically significant（ P〈0.05）. The T3 pain visual analogue score（VAS） in electric anesthesia group was significantly lower than that in general anesthesia group, the difference was statistically significant（P〈0.05）. The narcotic drugs dosage, adverse reaction rate in electric anesthesia group were significantly lower than those in general anesthesia group, the difference was statistically significant（P0.05）. [Conclusions]Transcutaneous electrical acupoint stimulation and intravenous general anesthesia can effectively improve the hemodynamics of patients undergoing gynecologic hysteroscopy, it is helpful to improve the analgesic effect of patients, and it can effectively reduce the narcotic drugs dosage and adverse reactions, it ＇s worth for further clinical promotion.
