光动力疗法联合超声龈下刮治术和根面平整术治疗重度慢性牙周炎的临床研究

摘　　要：目的比较光动力疗法(photodynamic therapy,PDT)联合超声龈下刮治术和根面平整术(Ultrasonic scaling and root planing,USRP)与超声联合手工龈下刮治术和根面平整术(Manual scaling and root planing,MSRP)治疗重度慢性牙周炎的临床疗效。方法重度慢性牙周炎患者20例,以自身为对照,每个患者选取牙周探诊深度(Probing depth,PD)≥7 mm的位点,随机分为实验组(USRP+PDT)和对照组(USRP+MSRP)。实验组在USRP后做一次光动力治疗,对照组在USRP后进行彻底地MSRP。对两组术前、术后1和3个月的探诊出血(bleeding on probing,BOP)、牙龈退缩(gingival recession,GR)、牙周探诊深度(Probing depth,PD)和牙龈出血指数(bleeding Index,BI)进行比较分析。结果术前两组相关数据比较,差异无统计学意义(P>0.05)。术后1和3个月两组患者的PD、BOP、BI和GR数值,与治疗前比较均有显著改变,差异具有统计学意义(P<0.05);实验组PD、BOP、BI和GR数值较对照组改善不显著,差异无统计学意义(P>0.05)。结论 PDT可作为牙周传统治疗方法的一种有发展潜力的辅助治疗手段。Objective To compare the clinical efficacy between combined application of photodynamic therapy（PDT） and ultrasonic scaling and root planning（USRP） and the combination of USRP and manual scaling and root planning（MSRP） for severe chronic periodontitis.Methods Totally 20 patients with severe periodontitis were randomly divided into the experimental group（USRP ＋ PDT） and control group（USRP ＋ MSRP）,each being a self-control. While the patients in the experimental group being given PDT after USRP,those in the control group were given MSRP after USRP. Probing depth（PD）,bleeding on probing（BOP）,gingival recession（GR） and bleeding index（BI） were recorded at baseline,1 and 3 months after the treatment.Results There was no statistically significant difference in PD,BOP and BI between two groups（P〉0. 05） before the periodontal treatment. For both groups,the clinical parameters improved significantly 1 month and 3 months after the therapy（P〈0. 05）. Nevertheless,the differences between two groups in the reduction of PD,BOP,GR and BI were not statistically significant（P〈0. 05）.Conclusions PDT is a promising therapy to assist traditional treatment of periodontal disease.
