摘  要:目的探讨青岛地区新生儿促甲状腺素(TSH)的筛查切值。方法对本市新生儿疾病筛查中心212 781例新生儿TSH结果进行分析,采用百分位数法及ROC曲线法制定合理的TSH切值,并分析采取不同切值确诊患者的情况。结果 212 781例新生儿TSH水平呈偏态分布,P25为1.15mU/L,P50为1.84mU/L,P75为2.93mU/L,P99为8.27mU/L,ROC曲线法得出本地区TSH筛查切值为8.13mU/L。若采用P998.27mU/L作为本地区TSH的切值,将漏诊2例CH患儿,漏诊率为2.11%。结论采用ROC曲线法确定最适宜的TSH切值,是提高先天性甲状腺功能减退症筛查质量的重要举措。Objective:In order to know the distribution of TSH and incidence of neonatal hypothyroidism(CH),and study the cutoff points of TSH in Qingdao. Methods:The dates of newborn screen centre of Qingdao were analyzed,which was 212 781 neonatals,calculated the TSH cutoff points by method of percentiles and ROC curve,and then,analyzed the complexion of sufferers by different cutoff points. Results:The distribution of the newborn screening data was deflection,P25 was 1.15 mU/L,P50 was 1.84 m U/L,P75 was 2.93 mU/L,P99 was 8.27 mU/L,combined with ROC curve,the cutoff points of our lab was 8.13 mU/L. If P998.27 mU/L is used as the cutoff points of TSH in Qingdao,the detection rate was missed by 2 case of CH,and the rate of missed diagnosis was 2.11%. Conclusions:The optimal TSH cutoff points was determined by ROC curve method,and it is important to improve the screening quality of congenital hypothyroidism.
