超声弹性成像技术对睾丸损伤的诊断价值

摘　　要：目的探讨超声弹性成像技术在诊断睾丸损伤中的应用价值。方法对2016年3月至2018年6月佛山市中医院收治的19例可疑睾丸损伤患者分别进行常规超声、超声弹性成像及超声造影检查,所有检查结果均与手术或随访结果对照。结果 19例患者中睾丸损伤确诊17例,合并附睾损伤2例,仅阴囊皮下组织挫伤2例。常规超声、超声弹性成像及超声造影诊断的灵敏度分别为94.1%、94.1%与100%,三者比较差异均无统计学意义(P>0.05)。超声弹性成像具有随睾丸损伤后不同时期而表现为由红色转变为蓝色的声像变化特征。结论常规超声、超声弹性成像技术及超声造影均能对睾丸损伤作出准确的定性诊断,超声弹性成像技术可评估患者的恢复情况,并作出有益的提示。Objective To evaluate the value of ultrasound elastography（UE） in the diagnosis of testicular trauma. Methods Conventional ultrasonography（US）, ultrasound elastography（UE） and contrast-enhanced ultrasonography（CEUS） were performed on 19 cases of suspected testicular injury admitted to Foshan Hospital of Traditional Chinese Medicine from March 2016 to June 2018, thereafter, the cases were confirmed by surgical operations or follow-up results. Results Seventeen cases out of 19 testicular traumas were confirmed, including 2 with epididymal injury, and2 with scrotal subcutaneous tissue contusion. The sensitivity of US, UE and CEUS were 94.1%, 94.1% and 100%, respectively. There was no significant difference in these methods（P〈0.05）. UE had the characteristics of changing from red to blue with different stages of testicular injury. Conclusion US, CEUS and UE can make accurate qualitative diagnosis of testicular injury. Ultrasound elastography can provide useful hints for the assessment of patients＇ recovery.
