免疫吸附疗法治疗急进性肾小球肾炎的临床研究

摘　　要：目的对患者的血清抗肾小球基底膜抗体(抗-GBM)、抗髓过氧化物酶抗体(ANCA-MPO),在治疗前后的变化进行分析,探讨免疫吸附疗法治疗急进性肾小球肾炎的临床效果。方法选取该院2017年4月—2018年4月收治的50例患者,观察患者治疗前后的抗-GBM、ANCA-MPO变化,进行免疫吸附疗法治疗急进性肾小球肾炎的效果研究。结果患者的抗-GBM与ANCA-MPO在治疗前后的对比明显,抗-GBM治疗前(22.68±22.71)EU/mL,治疗后(12.93±10.57)EU/mL;ANCA-MPO治疗前(86.78±45.49)EU/mL,治疗后(53.29±30.96)EU/mL,但是抗-GBM的差异无统计学意义(P>0.05),ANCA-MPO的,差异有统计学意义(t=5.02,P<0.05)。监测患者治疗前后的血压、呼吸变化、血氧饱和度以及心率都相对稳定。结论该临床研究显示,免疫吸附疗法治疗急进性肾小球肾炎,其治疗效果安全,可以有效的改善患者的病情状况,具有临床依据。Objective To analyze the changes of serum anti-glomerular basement membrane antibody （anti-GBM） and anti-myeloperoxidase antibody （ANCA-MPO） before and after treatment, and to explore the rapid advancement of im- munoadsorption therapy. The clinical effect of glomerulonephritis was explored. Methods 50 patients admitted to the hospital from April 2017 to April 2018 were enrolled. The changes of anti-GBM and ANCA-MPO before and after treatment were observed. The effect of immunoadsorption therapy on acute glomerulonephritis was studied. Results The anti-GBM and ANCA-MPO levels were significantly higher before and after treatment, before anti-GBM treatment （22.68_＋22.71）EU/mL, after treatment （12.93_＋10.57）EU/mL, before ANCA-MPO treatment （86.78_＋45.49）EU/mL, after treatment （53.29_＋30.96）EU/mL, for anti-GBM,the different was not statistically significant （P〉0.05）; for ANCA-MPO, the different was statistically significant（P〈0.05）. Monitoring blood pressure, respiratory changes, oxygen saturation and heart rate before and after treatment were relatively stable. Conclusion This clinical study shows that immunoadsorp- tion therapy for the treatment of rapid glomerulonephritis, its therapeutic effect is safe, can effectively improve the pa- tient＇s condition, and has a clinical basis.
