摘 要:目的分析宫颈上皮内瘤变(CIN)组织中人乳头瘤病毒(HPV)型别分布情况及其临床意义。方法从CIN组织中抽提HPV DNA,使用基因芯片结合聚合酶链式反应技术,对534例CIN组织标本行23种HPV检测,其中CINⅠ级病变157例、CINⅡ级病变138例及CINⅢ级病变239例,并对HPV感染者行型别分布状况的分析。结果 534例CIN患者中检出HPV感染者386例,总的HPV检出率为72.29%(386/534),其中CINⅠ级病变中总的HPV检出率为42.68%(67/157),CINⅡ级病变中总的HPV检出率为65.94%(91/138)、CINⅢ级病变中总的HPV检出率为95.40%(228/239)。CIN的程度与HPV检出率呈正相关。在CINⅢ级/原位癌病变中,16型出现率最高44.51%(154/346),随后是18、52、58、33和31型。结论对女性行宫颈HPV分型检测,有利于临床医师对宫颈癌前病变进行诊治和观察,也有利于跟踪和随访高致瘤致癌性的HPV感染者。Objective To compare the distribution states of human papilloma virus (HPV) genetypes in tissues of cervical intraepithelial neoplasia (CIN) and its clinical significance. Methods Polymerase chain reac tion (PCR) and gene chip technology were utilized for the detection of 23 kinds of genetypes in tissue speci mens from 534 cases of cervicalintraepithelial neoplasia,including 157 cases ofCIN Ⅰ grade,138 cases of CIN Ⅱ grade and 239 cases of CIN Ⅲ grade. HPV genetypes distribution states of all subjects were analyzed. Results In 534 cases of cervical intraepithelial neoplasia,the total detection rate of HPV was 72. 29% (386/ 534),the detection rate of HPV was 42. 68% (67/157)in CIN I grades,the detection rate of HPV was 65.94% (91/138)in CIN Ⅱ grades and the detection rate of HPV was 95.40% (228/239)in CIN Ⅲ grades. The cervical lesion degree was positively related to the detection rates of HPV. The detection rate of HPV 16 type is the highest and the present frequency of 16 type infection is 44.51% (154/346) in CIN Ⅲ grades,fol lowing types are 18,52,58,33 and 31. Conclusion HPV typing detection will benefit clinical gynecologist to diagnose,treat and follow up cervical precancerous lesion patients. The detection also benefits clinical physi clan on determining high carcinogenic HPV infected persons and conducting tracking follow up to the pa tients.
