精细化护理在内镜下硬化剂注射治疗食管胃底静脉曲张出血的应用体会

摘　　要：目的探讨精细化护理在内镜下硬化剂注射治疗食管胃底静脉曲张出血的应用体会。方法选取2015年7月至2017年12月我院收治的80例食管胃底静脉曲张出血患者为研究对象,随机将其等分为观察组和对照组。两组患者均采用内镜下硬化剂注射治疗,对照组患者给予常规护理,观察组患者在此基础上给予精细化护理,比较两组患者的止血效果、手术前后心率水平,干预前后焦虑自评量表(SAS)、抑郁自评量表(SDS)评分,健康教育知识掌握程度。结果观察组患者止血有效率优于对照组(P <0. 05);两组患者手术前心率水平比较差异无统计学意义(P> 0. 05),手术后心率水平低于对照组(P <0. 05);干预1个月后,两组患者SAS,SDS评分均较干预前降低,且观察组低于对照组(P <0. 05);观察组患者健康教育知识掌握程度优于对照组(P <0. 05)。结论对行内镜下硬化剂注射治疗的食管胃底静脉曲张出血患者采取精细化护理可改善止血效果,有利于维持手术过程心率稳定,减轻患者治疗过程中焦虑抑郁情绪,并能提高患者对健康教育知识的掌握程度,值得临床推广应用。Objective To explore the application of refined nursing to endoscopic sclerotherapy for esophageal and gastric varices bleeding. Methods To select 80 cases of patients with esophagogastric varices bleeding admitted to our hospital from July 2015 to December 2017 as the study subjects, and they were divided into observation group and control group randomly. Both groups of patients were treated with endoscopic sclerotherapy. The patients in the control group received routine nursing, while the patients in the observation group were given refined nursing. The hemostatic effect, heart rates before and after the intervention, self-rating anxiety scale（SAS） and self-rating depression scale（SDS） scores before and after the intervention, mastery of health education knowledge of the two groups were compared. Results The observation group was better than the control group in effect of hemostasis（ P ＜0.05）. There was no significant difference in comparison of preoperative heart rate between the two groups（ P 〉0.05）. The postoperative heart rate in the observation group was lower than that of the control group（ P ＜0.05）. After 1 month of intervention, both SAS and SDS scores of the two groups were decreased compared with that before the intervention, and the scores of the observation group were lower than that of the control group（ P ＜0.05）. The observation group had better knowledge of health education than the control group（ P ＜0.05）. Conclusion Applying refined nursing for patients with esophageal and gastric varices bleeding treated with endoscopic sclerotherapy can improve the hemostasis effect and maintain the heart rate stability during the operation. Meanwhile, it can reduce the anxiety and depression during the treatment, and improve the patient＇s mastery of health education knowledge,which was worthy of clinical promotion and application.
