腹腔镜下与开腹行全子宫或次全子宫切除术的疗效比较

摘　　要：目的:比较腹腔镜下与开腹行全子宫切除或次全子宫切除术的优缺点。方法:将2012年8月至2014年4月我院行子宫切除术的119例患者分为4组,腹腔镜下次全子宫切除术组(A组) 30例、腹腔镜下全子宫切除术组(B组) 28例、开腹次全子宫切除术组(C组) 21例、开腹全子宫切除术组(D组) 40例。结果:腹腔镜组患者比开腹组患者术后恢复快、住院时间短、术后疼痛减轻、抗生素使用时间缩短、肠道功能恢复快,但手术耗时延长,差异有统计学意义(P <0. 05);腹腔镜组与开腹组的手术出血量、术后最高体温差异无统计学意义(P> 0. 05)。结论:腹腔镜子宫切除术创伤小、恢复快,但对操作者技术要求高,易导致并发症发生。临床医师应根据患者的病情及操作者技术水平选择合适的术式。Objective： To compare the advantages and disadvantages between total/subtotal laparoscopic hysterectomy and total/subtotal abdominal hysterectomy. Methods： From August 2012 to April 2014, 119 patients receiving hysterectomy were divided into four groups： group A （receiving subtotal laparoscopic hysterectomy, 30 cases） , group B （receiving total laparoscopic hysterectomy,28 cases）, group C （receiving subtotal abdominal hysterectomy,21 cases） and group D （receiving total abdominal hysterectomy,40 cases）. Results： Compared with the laparotomy groups, the laparoseopie groups had faster recovery, shorter hospitalization time, less postoperative pain, shorter antibiotic use time and faster recovery of intestinal function, but the operation time was longer, the difference was statistically significant （P 〈 0. 05 ）. There were no significant differences in operative blood loss and postoperative body- temperature between laparoseopie groups and laparotomy groups （ P 〉 0. 05 ）. Conclusion ： Laparoseopie hysterectomy is a minimally invasive and quick recovery procedure, but it requires a high level of skill for the operator and is liable to lead to complications. Clinicians should select the appropriate operation according to the patient＇s condition and the skill level of the operator.
