摘 要:慢性肾脏病骨矿物质和代谢异常(CKD-MBD)是慢性肾脏病的常见并发症之一。其主要临床表现为继发性甲状旁腺功能亢进、钙磷代谢异常、骨骼成分与结构的改变、血管及软组织的钙化。CKD-MBD对患者的生存质量和长期预后有重要影响,其病理生理过程中的信号调控机制越来越受到关注,成为近年来医学研究的热点。其中,成纤维细胞因子23(FGF23)参与的钙磷代谢调节在CKD-MBD中起重要作用。且Klotho-FGF23轴调控Wnt/β联蛋白(β-catenin)信号通路在肾间质纤维化和CKD-MBD的发生、发展中均有重要作用。随着研究的深入,Klotho-FGF23轴调控Wnt/β-catenin信号通路有望成为CKD-MBD治疗的新靶点。The chronic kidney disease-mineral and bone disorder(CKD-MBD) is a common complication of chronic kidney disease.Its clinical manifestations include secondary hyperparathyroidism,metabolic abnormality of calcium and phosphorus,changes in bone composition and structure,calcification in blood vessels and soft tissues.CKD-MBD plays an important role in the quality of life and long-term prognosis of the patients.The signal regulation mechanism in the pathophysiological process has attracted more and more attention and become a hot spot of medical research in recent years.The regulation of calcium and phosphorus metabolism by fibroblast growth factor 23(FGF23) plays an important role in CKD-MBD.Furthermore,Klotho-FGF23 regulates Wnt/β-catenin signaling pathway, which plays an important role in the occurrence and development of renal interstitial fibrosis and CKD-MBD.With the deepening of the research,Klotho-FGF 23 axis regulation of Wnt/β-catenin signal pathway is expected to become a new target for the treatment of CKD-MBD.
