两种硬质三维成形环用于功能性三尖瓣反流的效果比较

摘　　要：目的观察佰仁思和爱德华MC3硬质三维成形环用于功能性三尖瓣反流(FTR)的治疗效果。方法选取2015年9月—2016年6月在解放军白求恩国际和平医院确诊的左心瓣膜疾病合并中度FTR 42例,按照应用三维成形环不同分为佰仁思组和爱德华组,每组21例。佰仁思组采用佰仁思三维成形环治疗;爱德华组采用爱德华硬质三维成形环治疗,观察并比较2组的手术效果、右心室前后径、肺动脉压力及并发症发生情况。结果 2组均无围手术期死亡。2组术后三尖瓣反流较术前明显降低(P <0. 05),但组间比较差异无统计学意义(P> 0. 05)。2组术后右心室直径及肺动脉压力均低于术前(P <0. 05),但组间比较差异均无统计学意义(P> 0. 05)。结论国产佰仁思三维成形环和进口爱德华MC3硬质三维成形环用于左心瓣膜病合并中度FTR效果相当,国产佰仁思硬质三维成形环值得临床推广。Objective To observe effects of Balmedic and Edward MC3 rigid three-dimensional （3-D） annuloplasty rings in application of surgery for functional tricuspid regurgitation （FTR）. Methods A total of 42 patients with left heart valve disease combined with moderate FTR admitted during September 2015 and June 2016 were divided into Balmedic and Edward groups （ n =21 in each group） according to types of 3-D annuloplasty rings. Balmedic group was treated with Balmedic rigid 3-D annuloplasty ring, and Edward group was treated with Edward rigid 3-D annuloplasty ring. Surgical effects, anteroposterior diameter of right ventricle, pulmonary arterial pressure and incidence rate of complications were observed and compared in two groups. Results There were no deaths during peroperative period in two groups. Degrees of FTR after surgery were significantly decreased compared with those before surgery in two groups （ P 〈0.05）, but the difference between two groups was not statistically significant （ P 〉0.05）. Values of anteroposterior diameter of right ventricle and pulmonary arterial pressure after surgery were significantly lower than those before surgery in two groups （ P 〈0.05）, but the differences between two groups were not statistically significant （ P 〉0.05）. Conclusion Domestic Balmedic and overseas Edward MC3 rigid 3-D annuloplasty rings may achieve similar clinical effects in the treatment of patients with left heart valve disease combined with moderate FTR.
