摘  要:目的对糖尿病(DM)患者体内微量元素水平进行检测,分析其临床意义。方法选取2016年3月至2017年3月该院收治的96例DM患者作为研究组,同期健康体检人群98例作为对照组。对两组患者的血清微量元素(铁、硒、铬、锌)、脂质代谢指标[总胆固醇(TC)、三酰甘油(TG)、载脂蛋白B(ApoB)、载脂蛋白A1(ApoA1)、载脂蛋白A2(ApoA2)]、糖脂代谢指标[C肽、糖化血红蛋白(HbA1c)、空腹血糖(FPG)]进行检测,并对其相关性进行分析。结果研究组血清铁、硒、铬、锌微量元素水平明显低于对照组,差异有统计学意义(P<0.05);研究组TC、TG、ApoB水平明显高于对照组,ApoA1、ApoA2水平显著低于对照组,差异有统计学意义(P<0.05);研究组C肽、HbA1c、FPG水平明显高于对照组,差异有统计学意义(P<0.05);Pearson相关性分析结果显示:铁、硒、铬、锌的水平均与TC、TG、ApoB、C肽、HbA1c、FPG水平呈负相关(P<0.05),且均与ApoA1、ApoA2水平呈正相关(P<0.05);多因素Logistic回归分析结果显示:硒、锌为DM患者的保护因素(P<0.05)。结论 DM患者体内存在微量元素铁、硒、铬、锌的异常情况,且其水平的变化与该病的发生、发展有着密切的关系,应加强对其微量元素的关注。Objective To detect the levels of trace elements in diabetic patients and analyze their clinical significance. Methods 96 patients with diabetes who were treated in hospital from March 2016 to March 2017 were selected as study group,98 healthy people during the same period were selected as control group.Serum trace elements (iron,selenium,chromium,zinc),lipid metabolic markers (TC,TG,ApoB,ApoA1,ApoA2),glucose and lipid metabolism markers (C-peptide,HbA1c,FPG) were detected in both groups of patients,and analyze their relevance. Results The serum iron,selenium,chromium,and zinc levels in the study group were significantly lower than those in the control group ( P 〈0.05).The TC,TG,ApoB levels in the study group were significantly higher than those in the control group,and the ApoA1,ApoA2 levels in the study group were significantly lower than those in the control group ( P 〈0.05).The C-peptide,HbA1c,FPG levels in the study group were significantly higher than those in the control group ( P 〈0.05).Pearson correlation analysis showed that the levels of iron,selenium,chromium,and zinc were significantly negatively correlated with the levels of TC,TG,ApoB,C peptide,HbA1c,and FPG ( P 〈0.05),and they were all have significant positive correlation with ApoA1 and ApoA2 ( P 〈0.05).Multivariate logistic regression analysis showed that selenium and zinc are protective factors for patients with diabetes. Conclusion There are abnormalities of trace elements iron,selenium,chromium,and zinc in patients with diabetes,and their changes are closely related to the occurrence and development of the disease.Therefore,attention should be paid to trace elements in the prevention and treatment of diabetes.
