摘　　要：目的研究多囊卵巢综合征(PCOS)妇女克罗米芬抵抗状况,指导合理用药。方法选取2015年1月至2017年1月该院收治的121例PCOS患者为研究对象,按照患者是否抵抗克罗米芬将所有患者分为抵抗组和对照组。比较两组患者年龄、体质量等一般资料、并发症发生情况、免疫功能及糖代谢相关指标。结果抵抗组患者的体质量、体质量指数(BMI)、舒张压及双卵巢窦卵泡数分别为(66.47±11.94)kg、(26.79±3.95)kg/m^2、(77.05±8.46)mm Hg、(38.05±7.99)个,痤疮及多毛症发病率分别为15.22%、10.87%,C反应蛋白、空腹胰岛素及胰岛素抵抗指数分别为(0.39±0.56)mg/dL、(14.96±11.64)μIU/mL、(3.68±3.21)个,与对照组患者比较均明显升高,差异有统计学意义(P<0.05)。结论体质量、BMI、舒张压、双卵巢窦卵泡数、痤疮、多毛症、C反应蛋白及空腹胰岛素水平可能是PCOS患者克罗米芬抵抗的临床指标。Objective To study on the resistance of clomifene citrate capsules in women with polycystic ovarian syndrome,in order to avoid the ineffectively use of clomiphene-resistant patients. Methods A total of 121 polycystic ovarian syndrome patients from January,2015 to January,2017 in our hospital were selected All patients were divided into resistance group and control group according to whether the patient was resistant to clomiphene. Results In the resistance group,the body weight,body mass index,diastolic blood pressure,and number of ovarian follicular follicles were （66.47±11.94）kg,（26.79±3.95）kg/m 2,（77.05±8.46）mm Hg,（38.05±7.99） respectively,the acne and hirsutism incidence were 15.22%、10.87% respectively,and the C-reactive protein,fasting insulin and insulin resistance index were （0.39±0.56）mg/dL,（14.96±11.64）μIU/mL,（3.68±3.21） respectively,which were significantly higher than those in the control group （ P 〈0.05）. Conclusion Body weight,body mass,diastolic blood pressure,number of ovarian follicles,acne,hirsutism,C-reactive protein and fasting insulin levels are likely to be clinical indicators of clomiphene resistance in patients with polycystic ovary syndrome,which should get special attention to avoid the ineffectively use of clomiphene-resistant patients.
