结肠充盈超声在诊断结肠癌及炎症性肠病中的初步探讨

摘　　要：目的分析大肠充盈超声、X线造影及CT检查对诊断Crohn病(CD)、溃疡性结肠炎(UC)及大肠癌的特异度和灵敏度。方法选取在我院病理确诊的炎症性肠病及大肠癌患者,该99例患者均行肠镜、大肠充盈超声、X线造影、及CT检查。收集临床影像资料。通过统计学分析,比较这三种检查方法对这三种疾病诊断的特异度和灵敏度。结果充盈超声、CT及X线造影检查对诊断大肠癌的灵敏度分别为:86.3%、68.2%、81.2%,特异度分别为:87.3%、74.5%、80.0%,均无统计学显差异(P>0.05),但对于溃疡性结肠炎及Crohn病的诊断充盈超声检查和X线造影检查明显优于CT检查,差异有统计学意义(P<0.05)。结论针对大肠癌的诊断,充盈超声、X线造影及CT检查其准确性无明显差异,但对于溃疡性结肠炎及Crohn病的诊断充盈超声和X线造影检查明显优于CT检查。Objective Investigate the diagnosis sensitivity and specificity of these three methods for diagnosis of Crohn disease,ulcerative colitis（UC）and CRC.Methods From 99 patients with IBD or CRC were enrolled.This patients were all examinated with colonoscopy,bowel contrast ultrasonography,X-ray radiography and computed tomography（CT）.The clinical image data were collected.Results The sensitivity of bowel contrast ultrasonography,CT and X-ray angiography in the diagnosis of colorectal cancer were：86.3%,68.2%,81.2%,specificity were：87.3%,74.5%,80.0%,there was no statistically significant difference（P〉0.05）.But in the diagnosis of UC and CRC,bowel contrast ultrasonography and X-ray angiography were much better than CT（P〈0.05）.Conclusions For diagnosis of CRC,there was no significant difference in accuracy among ultrasonography,X-ray radiography and computed tomography（CT）.But for diagnosis of UC and CRC,ultrasonography and X-ray angiography were much better than CT.
