摘  要:目的通过婴儿髋屈曲外展位髋内侧超声声像图表现,探讨在婴儿发育性髋关节发育不良(DDH)挽具治疗中,应用超声评估股骨头复位情况的临床应用。方法收集在我院就诊32例单侧髋关节脱位的DDH患儿,即患侧经Graf法诊断为Graf D、Ⅲ、Ⅳ型,健侧诊断为Graf I型。应用挽具治疗,髋屈曲外展位髋内侧超声观察髋关节解剖结构,评估股骨头与髋臼的位置关系。结果通过超声检查单侧髋关节脱位患儿的健侧髋,即挽具治疗前超声诊断为Graf I型,展示髋屈曲外展位髋内侧超声的正常髋关节相关解剖结构及声像图表现。同时,规定具体操作步骤及其超声切面,提高检查的可重复性。结论佩戴挽具治疗婴儿DDH的过程中,髋屈曲外展位经髋内侧行超声检查可以清晰显示并识别髋关节相关结构,有助于临床评估股骨头与髋臼的对位关系。Objective To show the sonographic anatomy of the infant flexed and abducted hip on an interior ultrasound,and to explore the value of this approach in harness treatment of DDH.Methods Since,32 patients with unilateral hip dislocation were treated by Pavlik harness.In the follow-up of treatment,results of reduction were assessed by hip interior ultrasound in the combining flexion and abduction position.All the patients were examined by Graf method before treatment.The affected hip is Graf D,Ⅲ,Ⅳtype.Results According to the unaffected hip which was proven as Graf I type before treatment,the normal anatomy and sonographic feature of hip joint was shown and described.Manipulating procedure and ultrasound images were illustrated.Conclusions In the follow-up of DDH infants undergoing harness treatment,interior ultrasound can display hip structure.It is useful to assess reduction results.
